SERVICE AGREEMENT

This contract has been entered on the 04.02.2009 by and between XXXXX Pvt. Ltd
represented by Director / Chief Operating Officer, Mr. XXXXX hereinafter shall be
called as “employer”.
AND
Ms. yyyyy, D/o John K.I, aged about 21 Years, Kadaliparambil (House), Veliyanad
(Post), Arakkunnam (Via), Ernakulam (District), Kerala (State) hereinafter shall be called
the “employee”.

The employee and employer have hereby agreed that the employee shall work at
Mediciti Healthcare Services Pvt. Ltd as Staff Nurse in the department of Nursing from
08th of February 2009.

The employee appointed as Staff Nurse in the department of Nursing in this
organization w.e.f. 08.02.2009 agrees that she/he shall work for a period of Minimum
TWO years starting from 08.02.2009.

The company has appointed the employees in the category of Probation initially for a
period of Six months, likely to be extended for a further period depending upon the
performance in acquiring the knowledge in this XXXX Pvt. Ltd. hospitals.

On this day of 09.02.2009, the employee hereby undertakes to serve the company in full
conscience and with his/her own free will. The employee has agreed as follows:-

1. To serve the company for a period of TWO years as per the agreed terms and
conditions.
2. To obey and abide by all the rules, regulations, service conditions and code of
conduct of the company and of its Authorized officers and representatives issued
form time to time.
3. The employee shall compensate the company the expenses incurred for imparting
the training and specialty skills, and also towards liquidates damages for non-
compliance of this agreement.
4. Termination of services by the company for whatever reason including for non-
compliance of, as per agreed terms and conditions, and also on medical grounds.
5. The company shall have the right to recover as its discretion the compensation
due to it from the employee.
6. The company shall not be responsible for any interruption in or stoppage of
learning as well as service of the employee due to exigencies of work or for
causes beyond its control.
7. The other conditions of appointment of the employee with the company as
expressly modified hereby shall remain unchanged and shall continue to be
binding on the employee.

8. The employee shall not break this agreement due to any reason whatsoever. In
case the employee breaks the bond in between the employee shall have to pay an
amount of Rs.10,000/- (Rupees Ten Thousand only). In case of non–compliance
due to any reason, the management can take necessary action against such
employee.
9. The employee agreed that he/she will not indulge in any activities which shall
harm the reputation of the company or damage the property of the hospital
10. The employee agreed that he/she will not disclose any information, which he/she
obtained from his/her activities on jobs to any other person, except to the
Administrator.

The employee has understood and agreed to the above in his/her full conscience and
with free will under NO pressure.
