Not all items on this checklist may apply in your situation. Taxes can be very complex and may require additional information. Please make me aware of any situation that has changed in your life that may have tax consequences. Please contact me with any questions you may have or if you need further instructions on required documentation. I look forward to the opportunity to work with you this year.

1.) PERSONAL INFORMATION


Your full name, social security number, and date of birth. Your spouses full name, social security number and date of birth. Your dependents & childrens full names, social security numbers and dates of birth. Copies of last years tax return (New Clients Only) Bank account number and routing number (Only if depositing your refund directly into your account).

2.) OTHER INFORMATION


Any Childcare records (including the providers tax ID number) where applicable. The approximate income of other adults in your home (however, not your spouse if youre filing jointly). If applicable, Form 8332, and copies of your divorce decree, or other documentation showing that your exspouse has waived their right of claiming a child to you. If applicable, amount of any alimony paid and ex-spouses social security number.

Wage Statements from Employer (Form W-2)

4.) SAVINGS AND INVESTMENTS


All relevant Interest, dividend income (Forms 1099-INT, 1099-OID or 1099-DIV). All relevant Income from the sale of stock or other property (Forms 1099-B or 1099-S). All relevant Dates of acquisition and records of your cost basis or other basis in property you have sold throughout the year.

5.) RENTAL INCOME


All records of income and expenses pertaining to Rental property. All Rental asset information so that it may be depreciated over the tax year. For example - cost, date placed in service, etc.

6.) RETIREMENT INCOME


All relevant Pension/IRA/annuity income (Form 1099-R). All relevant Social security/RRB income (Forms 1099-SSA or RRB-1099).

7.) IRA INFORMATION


Amount contributed for 2012 (and 2013, if applicable). Traditional IRA applicable basis. Value of IRAs as of Dec. 31, 2012.

8.) SELF-EMPLOYMENT INFORMATION


Form 1099-MISC, Schedules K-1, or other income records to verify amounts not reported on 1099s. Records of all expenses - check registers or credit card statements and valid receipts. Business-use asset information (cost, date placed in service, etc.) for depreciation. Home Office deduction information (Contact me for further details). Vehicle Costs such as: o Method 1: Standard Mileage Rate Method: Account of your TOTAL MILES driven for the year (you can also use your odometer reading from the beginning of the year and the end). o Account of your Total BUSINESS MILES driven for the year. Account of all parking and tolls fees that were paid (with receipts). Commuting travel to and from your primary place of business do not count as business miles.

Method 2: Actual Expenses You must have all receipts and/or totals for gas, oil changes, car washes, license fees, personal property tax information, lease or interest expense fees, etc.

9.) OTHER INCOME


Any Unemployment Income received or State Tax Refunds granted to the filing party(s) (1099-G). Any Gambling income earned by the filing party(s) (W-2G or records showing income, as well as expense records) NOTE this income can be offset by Gambling Losses with proof of loss (but losses can ONLY be claimed when claiming Winnings). Any Amount of alimony monies received and ex-spouses name for record. Any Health care reimbursements (1099-SA or 1099-LTC). Any Jury duty records. Any Hobby income and expenses in which to offset the income. Any monetary Prizes and awards earned on behalf of the filing party(s). Any Other 1099 forms showing earnings in the tax year.

10.) EDUCATION PAYMENTS


Bills from the educational institution or anything else that itemizes what you paid or received loans for versus what was covered by scholarship or other financial aid. Itemization of expenses incurred for higher education (Books, supplies, etc.). Forms 1098-T and 1098-E, if you received them. Scholarships and fellowships.

11.) ITEMIZING DEDUCTIONS


Forms 1098 or other mortgage statements showing applicable Interest paid. All Amounts of state/local income tax paid (other than wage withholding), or amount of state and local sales tax paid in advance by the filing party(s). Real estate and personal property tax records for payments made. Invoice showing amount of vehicle sales tax paid for the filing party(s). HUD statement showing closing date of home purchase in order to write off qualifying new home expense. Cash amounts for any Donations made to churches, houses of worship, schools, other charitable organizations, with applicable receipts. Receipts and Records of non-cash charitable donations to places such as Goodwill, etc. Any amounts paid for healthcare insurance and fees not covered by insurance that were paid to doctors, dentists, hospitals and other medical place. Account of all miles driven for charitable or medical purposes on behalf of filing party(s). Any expenses related to your investments. Amount paid to your tax professional for the preparation of your 2011 tax return. Employment-related expenses (Union dues, publications, tools, uniform costs and cleaning, travel costs, other related employee expenses not reimbursed by the Employer). Job-hunting expenses.
