Important Information: Consular Closings The Consulate of China in Los Angeles will be closed from Monday, October 1 until Wednesday, October 3, 2012. Two Year Visas Two Year, Multiple Entry visas are only issued to former Chinese nationals. Eligible applicants wishing to apply for this visa must appear in person at their local Chinese consulate.

You Need To Provide: Passport You must provide your actual signed passport, including one copy of the personal information page of your passport. Your passport must: q Be valid for six months beyond the length of your requested visa (for example, if you are requesting a 12 month visa your passport must be valid for 18 months ) q Have at least one blank visa page (amendments and endorsement pages cannot be used to fulfill this requirement) q Not be frayed, torn, separating, or altered in any other way If your passport does not meet these requirements, please contact WorldStrides at 1-800-422-2368 for further details. Photographs You must provide one color, passport-type photograph. The photograph must: q Be professionally taken q Be printed on photo paper q Be two inches by two inches q Have a white background q Have been taken within the last six months NOTE: No eyeglasses are to be worn for the photographs. Proof of Travel Arrangements To fulfill the Tourist visa requirement, your visa application must provide proof of travel arrangements during the duration of your trip including: q Your hotel confirmation (provided by WorldStrides) q Your ticketed flight itinerary (provided by WorldStrides, if applicable*) *If you booked your own travel, please provide confirmed flight information to Visa Coordinator Jessica Cue. Driver's License You must provide a copy of your driver's license: Your copy must display your home address as it appears on your visa application form.

Visa Application Requirements

You Need To Fill Out: Visa Application Form You must provide one completed copy of the visa application form found in this kit. The application form must: q Include answers for all fields q Display your full name as it appears in your passport q Be signed q If you are employed, you must state the title of your place of employment on page 1, number 1.13 under Other. You must be specific.

This May Pertain To You: Applicants Born in China, Taiwan or Hong Kong You must write your name in Chinese on the application form. If you have never been issued a Chinese visa then you must send your original Chinese, Taiwanese or Hong Kong Passport with your other supporting visa materials. Your original passport will be returned to you. If you have been issued a Chinese visa, you must provide a copy of the previous Chinese visa. Hong Kong or Macau Travel Visiting Hong Kong or Macau is considered leaving China. You must obtain a multiple entry visa if you leave Mainland China to visit Hong Kong and Macao with the intention to re-enter the mainland before finally departing from China. Children 12 and Under You must provide the original and a copy of your birth certificate. For minors who are not traveling with one or both parents, you must include a photocopy of a notarized letter of consent to travel signed by your parents who are not traveling and include a clear photocopy of the parent's drivers license or passport information page. The minor must travel with the original notarized consent letter.

By placing an order with WorldStrides and CIBT, you agree to the following terms and conditions. WorldStrides and CIBT will make every effort to provide you with accurate information and to obtain the travel visa, passport or other documents required for your international trip. From time to time, consulates may change their requirements without advance notification. In the event that this occurs, you will be contacted immediately to obtain any new documents required for the successful completion of your travel document needs. Processing of travel visas often requires several days, and in some instances can take several weeks. It is the participants responsibility to submit all required documentation to WorldStrides and CIBT prior to the deadline communicated by WorldStrides and CIBT; failure to do so may result in the participants inability to travel. You are responsible to verify that all required travel documents have been obtained, that the personal details therein are accurate and that your travel documents are valid for at least 6 months beyond the completion of your trip. The government agency issuing a visa or passport (issuing authority) has complete discretion over its processes and may reject a travel document application for any reason or no reason. The issuing authority will make the final determination as to the type of visa or passport, how quickly it will be issued and for what duration it will be issued. Prior to approval, the issuing authority may ask for additional documentation. Any fees paid to a government agency are non-refundable. Please note that, even when a visa is issued, a traveler may be denied entry since in each country the local immigration officials make the final entry decision. It may be possible to obtain a required visa upon arrival at some of these destinations. Significant restrictions often apply and requirements for entry vary. Check with the local consulate for details. Inquire with your airline if boarding is allowed without a visa. Immigration officials in the destination country can refuse entry to anyone for any reason. WorldStrides and CIBT accept no liability for travelers electing to obtain on-arrival visas. WorldStrides and CIBT recommend that participants purchase travel insurance in the event that you are unable to travel with the group. WorldStrides programs are generally non-refundable. Purchasing trip insurance may help protect your trip investment in the event you are denied entry into a destination country. In the event that you are denied a visa, your WorldStrides Account Manager may attempt to recoup funds on your behalf; however, it is not guaranteed. Please reference the terms and conditions that you acknowledged when registering to travel with WorldStrides. Liability for any claims, causes of action or damages that you may assert shall be limited to the lesser of the travel document service fees paid to WorldStrides or $500. WorldStrides and CIBT will not be liable under any circumstance to compensate you for any loss, damage or delay of travel documents or other failure or fault in performance by any third party deliver company such as FedEx, UPS, DHL/Airborne, USPS, local delivery services and other delivery methods. Likewise, by submitting an order to WorldStrides and CIBT you agree that WorldStrides and CIBT will not, under any circumstances, be responsible for the loss, damage, or delay in returning of passport and documents by the consulates or passport agency, nor be liable for the failure of the consulate/embassy or passport agency to issue requested documents on time or at all.
