1. Purpose. This Directive implements policy for responsibilities, and procedures for the issuance of
Identification cards to members of the New York Guard. Technical specifications and issuing
procedures for Identification cards are outlined in Chapter III.

2. Application

a. This Instruction applies to all members of the New York Guard.

b. This publication is directive in nature. These formats, and procedures will be followed, except
when, in the judgment of the commander, exceptional circumstances dictate otherwise.

3. Scope. This publication describes the procedures for issuing Identification Cards to members of the
New York Guard.

4. Basis. DoD Instruction 1000.1, "Identity Cards Required by the Geneva Conventions," January

CHAPTER I

POLICY

It is New York Guard policy to provide members of the NYG with a distinct ID card for use in
identifying their status as an Active Duty, Reserve, or Retired member of the New York Militia Forces.

CHAPTER II

RESPONSIBILITIES

1. Commander, New York Guard. Approving authority for NYG Form 1000 policy.

2. Adjutant General, NYG. The Adjutant General is responsible for the physical design of the NYG
Form 1000, and acquisition of all production hardware and software.

3. The J-1, Deputy Chief of Staff for Personnel and Administration will;

a. Establish overall Command responsibility and procedures for the issuance of NYG Form 1000
to active members of the NYG, and other eligible individuals.

b. Ensure training is conducted and only qualified operators utilize card production equipment.

4. The J-4, Deputy Chief of Staff for Logistics will establish overall policy and procedures for the
hand receipt of NYG Form 1000 production hardware and software to using units.

5. Commanders, New York Guard Components. Commanders of the Army Division, Air Division
and Civil Military Affairs Unit will;

a. Ensure NYG Form 1000 is issued to members IAW this Directive.

b. Ensure that when new NYG Form 1000 is issued, or when members are discharged, that
obsolete cards are recovered and destroyed.

6. Identification Card Machine Operators. Operators will;

a. Ensure NYG Form 1000 is issued only to eligible members IAW this Directive.

b. Ensure that when new NYG Form 1000 is issued that obsolete cards are recovered and
destroyed.

c. Not issue cards that modify eligible member’s rank, status, or ETS/MRD.

CHAPTER III

PROCEDURES

1. Card Description. The NYG Form 1000, “Armed Forces of the State of New York, New York
Guard Identification Card” is a composite 80\20 PVC\Poly card, ISO CR-80 – ISO 7810, 2.125
inches (54 mm) x 3.385 inches (86 mm), of .030 inches thickness. The card face is printed in color
with a light pink tamper proof pattern, colored New York State Seal, color photograph, and colored
NYG Eagle. The face also contains the individual’s name, signature, rank, ID number and expiration
date. The card reverse is printed in black and contains personal data and characteristics with a Code
39 Bar Code. Any over printing or modification to the card renders it invalid.

2. Card Issue. The following criteria apply to the use and issuance of the NYG Form 1000, “Armed
Forces of the State of New York, New York Guard Identification Card” (Active/Reserve/Retired).

a. Active Members. The NYG Form 1000, “Armed Forces of the State of New York, New
York Guard Identification Card” is the primary ID for active duty NYG members and shall be used to
identify the member's eligibility for access to NYS Military facilities. The manually prepared laminated
card [DMNA Form 2 (NYG) JUL95], is being replaced by the NYG Form 1000, “Armed Forces of
the State of New York, New York Guard Identification Card” card. The use of other media is not
authorized.

b. Reserve Members. The NYG Form 1000, “Armed Forces of the State of New York, New
York Guard Identification Card” may be issued to Reserve Members at the discretion of the
Commander. Members who have voluntarily been placed on reserve due to temporary medical,
employment or residence location problems, who may be available for recall to duty, may be issued a
card. Members involuntarily placed on reserve status will not be issued a reserve card.

c. Retired Members. The NYG Form 1000, “Armed Forces of the State of New York, New
York Guard Identification Card” may be issued to Retired Members at the discretion of the
Commander. Members who have active service up to mandatory retirement, and remain active

on Retired Advisory Boards or in Regimental Affiliations may be issued a card. Also retired officers
who have been brevetted.

d. Dependents. The “Armed Forces of the State of New York, New York Guard Identification
Card” is not authorized for issue to dependents of members.

e. Recall Cards. NYG Form 1000, “Armed Forces of the State of New York, New York Guard
Identification Card” shall be issued to members of the NYG serving on, or recalled to active duty in
excess of 30 days. Members who are ordered or recalled to active duty for periods of 30 days or less
shall not be re-issued an ID Card, but shall be identified by copies of their orders or other documents
establishing temporary active duty status and by their NYG Form 1000 (Reserve) or NYG Form 1000
(Retired), as appropriate.

3. Re-issuance.

a. Lost or Stolen. A NYG Form 1000, “Armed Forces of the State of New York, New York
Guard Identification Card” shall be replaced when lost or stolen.

b. Change in printed data. A NYG Form 1000, “Armed Forces of the State of New York, New
York Guard Identification Card” shall be replaced when printed information requires changes, such as a
change in rank, as supported by database information.

c. Defaced. A NYG Form 1000, “Armed Forces of the State of New York, New York Guard
Identification Card” shall be replaced when any of the media (to include printed data, or bar code)
becomes illegible or inoperable with card readers.

4. Retrieval and Destruction. Invalid, inaccurate, inoperative, or expired NYG Form 1000,
“Armed Forces of the State of New York, New York Guard Identification Cards” shall be returned to
the individual’s unit for disposition. Individuals must turn-in their old card for issuance of a replacement
card. The individual responsible for issuing the new card will destroy the old card by cutting it up.

5. Use.

a. The card, which is the property of New York State, shall be in the personal custody of the
member at all times. It shall be provides to military authorities for identification, investigation, or while
the member is in military confinement.

b. Any person willfully altering, damaging, lending, counterfeiting, or using these cards in any
unauthorized manner is subject to dismissal. Photographing or otherwise reproducing or possessing a
NYG ID card in an unauthorized manner is subject to dismissal.

c. Unauthorized or fraudulent use of the NYG Form 1000, would exist, if a person uses the card
to obtain benefits and privileges to which he or she is not otherwise entitled.

d. The NYG Form 1000 shall not be posted or shown on web sites, or shown actual size. Any
photocopy will always be shown with “SAMPLE” on it. The likeness shown within this document will
be used to the maximum extent possible.

e. The NYG Form 1000 shall not be amended, modified, or overprinted by any means. No
stickers or other adhesive materials are to be placed on either side of the NYG Form 1000. Holes shall
not be punched into the NYG Form 1000.

6. Access.

a. The NYG Form 1000, “Armed Forces of the State of New York, New York Guard
Identification Card” is the primary ID for active duty NYG members and shall be used to identify the
member's eligibility for access to NYS Military facilities.

b. The NYG Form 1000, “Armed Forces of the State of New York, New York Guard
Identification Card” does not guarantee access to Federal Installations. Access is based on local
policies of the federal installation and additional authorization, such as orders, may be required.

7. Benefits.

a. This card does NOT authorize the member any eligibility for medical benefits or commissary
privileges. This card does not, by itself, authorize exchange, commissary, or morale, welfare, and
recreation privileges. Additional authorization, such as orders, are needed for access to Federal Post
Exchanges (PXs) where there are local agreements with federal military installations.

b. The NYG Form 1000 will be required for meal headcounts during annual training and other
subsistence supported events.

8. NYG ID Card Production.

a. Database. The automated system will enter the appropriate information in the appropriate
blocks based on the information contained in the NYG database. The database provides Name, Rank,
ETS/MRD (Expiration Date), NYG ID#, ID Card Number, Date of Birth, Weight, Height, Hair Color,
Eye Color, Blood Type, and Category.

b. Name. The individual’s name will be entered as shown in the database. If there has been a
change, such as due to marriage, verifiable documentation will be provided to update the database prior
to issuance of a card.

c. Rank. The individual’s rank will be entered as shown in the database. If there has been a
change due to unit promotion, verifiable documentation will be provided to update the database prior to
issuance of a card. The operator IS NOT AUTHORIZED to issue a card at other than the listed rank.

d. Photograph. The photograph will be a full face photo, in color, comprising at least 80% of the
image area. The background may be white, off white, light gray, or light blue gray in color.

(1) On site issue. That block shall contain a digitized, full-face photograph generated from
the digital camera peripheral.

(2) Off site request. A color digital photograph 640 pixels x 480 pixels in JPG format will be
forwarded with a written request for issuance of ID Card. The background must include the area
behind the head to include shoulders. The individual’s face must comprise at least 1/6th of the image.

(3) Re-Issue. The previous photograph on file will be used.

e. Signature.

(1) On site issue. That block shall contain the signature of the cardholder generated from the
Signature Pad peripheral.

(2) Off site request. To be announced, system requires a WMF file.

(3) Re-Issue. The previous signature on file will be used.

f. Expiration Date. The "expiration date" shall be shown, as prescribed. Enter all dates in the
four-digit year, two-digit character month, and two-digit day format (YYYYMMDD).

(1) Active Officer. The "expiration date" shall be shown as the age 68 Mandatory Removal
Date (MRD).

(2) Active Enlisted. The "expiration date" shall be shown as the End of Term of Service
(ETS). NOTE: if the database ETS has past, NO card will be issued until proof of re-enlistment is
provided and the database corrected.

(3) Retired. The "expiration date" shall be shown as Indefinite.

(4) Reserve. If a reserve card is authorized, the "expiration date" shall be shown as the age
68 Mandatory Removal Date (MRD).

g. NYG ID#. The New York Guard ID number is entered from the database. This field will not
be altered.

h. ID Card Number Bar Code. The New York Guard ID Card number is generated from the
database. This number uniquely identifies the card and is used to manage ID Card processing. This
field will not be altered.

i. Date of Birth. The date of birth is entered from the database. This field will not be altered. The
date will be entered in the four-digit year, two-digit character month, and two-digit day format
(YYYYMMDD).

j. Weight. The individual’s weight is entered from the database if that information has been
provided. If missing this information will be entered, and a batch program will provide updated
information to the NYG database. The weight is in pounds and will be entered in whole numbers.

k. Height. The individual’s height is entered from the database if that information has been
provided. If missing this information will be entered, and a batch program will provide updated
information to the NYG database. The height is listed in feet (‘) and inches (“).

l. Hair Color. The individual’s hair color is entered from the database if that information has been
provided. If missing this information will be entered, and a batch program will provide updated
information to the NYG database. Enter basic colors only, do not abbreviate;

(1) AUBURN

(3) BLONDE
(4) BROWN
(5) GRAY
(6) RED
(7) WHITE
(8) BALD. (don’t use unless medical condition has rendered individual hairless)

m. Eye Color. The individual’s eye color is entered from the database if that information has been
provided. If missing this information will be entered, and a batch program will provide updated
information to the NYG database. Enter basic colors only, do not abbreviate;

(1) BLUE
(2) BLACK
(3) BROWN
(4) GRAY
(5) GREEN
(6) HAZEL
(7) VIOLET

n. Blood Type. The individual’s blood type is entered from the database if that information has
been provided. If missing this information will be entered, and a batch program will provide updated
information to the NYG database.

o. Category. The category field will be populated from the database as one of the following;

(1) ACTI. Indicates an active member.


(2) RETI. Indicates a retired member, past age 68.
(3) RESE. Indicates a Reserve List Officer who may be recalled.

p. Date of Issue. The date of issue will automatically be entered by the computer. The date will
be entered in the four-digit year, two-digit character month, and two-digit day format, separated by
“slashes” (YYYY/MM/DD). The date time group of issue, and operator, will be included in the batch
program update to the NYG database.
