Learning Outcomes - in completing these tasks you will learn to: 1. Prepare an appropriate health screening questionnaire (P2) 2. Devise and use appropriate health screening procedures for two contrasting individuals (P3) 3. Safely administer and interpret the results of four different health monitoring tests for two contrasting individuals (P4) 4. Describe the strengths and areas for improvement for two contrasting individuals using information from health screening questionnaires and health monitoring tests (M2) 5. Evaluate the health screening questionnaires and health monitoring test results and provide recommendations for lifestyle improvement (D1) Task(s), indicating the P, M, D criteria reference.

Design a Health screening questionnaire and informed consent form. Then complete the questionnaire and consent form with 2 new clients and use the information gathered to provide verbal feedback to your clients regarding their suitability for sports participation. Finally, administer appropriate health monitoring tests on your clients before interpreting their results and giving them written feedback about their results. Questionnaire and Consent form. (P2) This will enable you to assess the current lifestyle choices of your client and ensure that they understand the implications of the testing they are about to complete. Client feedback. (P3) This will require you to perform an interview with your client where their health screening results are interpreted by you and you give appropriate feedback about their current lifestyle choices.

Health monitoring tests. (P4, M2, D1) You will be required to select and administer four appropriate health monitoring tests. You must then interpret the results gained and complete a written report explaining your interpretations and making recommendations for future lifestyle decisions. Your report should include a detailed analysis that compares the clients scores to normative data. This analysis will then provide the basis for some detailed feedback to your client regarding their lifestyle choices. You might make suggestions that relate to Physical activity or healthy eating, for example.
