Oiler B was injured while burning Oily rags in Incinerator due to lack of SMS matter. Also, Ash removal doors were out of alignment. Non-conformity is existed on ISM Matter. Port side Lifeboat was 2 exposing places on hull/keel forward. Ship is using 2 Oil Record Books, one is Part I and other is Part III. The some Code number is mixed up for the both Oil Record Book. Non-conformity for more understanding / training. Various Sea Water Pipe Lines, Steam Pipe Lines and Fresh Water Pipe Lines had many patches/leaks. Ship installed the temporary collection Drum with flexible hose from Main Air Compressor and Main Starting Air Bottles. There is no proper handrails from the entrance to the local control area in Steering Gear Room. Hydraulic oil is leaking from Ram Cylinders. Both Lifeboats Steering Unit is inoperable.
