FOUNDATIONS BACKGROUND The Project MAMBILLA Contractor (Vita Construction Limited) needs pile at the making to protect the boilers foundations WORK SUPERVISION AND DURATION: 1 the work is proposed to start on 19th December, 2012 2 Supervision will be done by Mambilla Project civil project manager Mohammed WORK DESCRIPTION Barricade of the approved area with flexible barrier caution tape and latter by rigid barrier wood and zinc The area has been marked Demolition of the breaking of wall will be done manually with sledge/mason hammer and jack hammer (wall wail be demolish from the top to the bottom and cart away Clean up the broken concrete to lay down area with dumper and latter removed with tipper out of the plant Break the concrete floor and clean up Set up the pilling machine Drill to 3.5mdepth by 450mm diameter ( the drilled sand comes up as the drilling progresses) Cutting and tying of reinforcement will be done at lay down area Mix the concrete slurry and pour into the drilled hole Fix the reinforcement and push it into the concrete slurry This process is repeated for the rest of the piles Site cleaning Removal of hoarding

WORK SUPERVISION AND DURATION The contractor (Vita Construction Limited) is familiar with the scope of the work Any change will be communicated within 24hours ahead of implementation. Supervision will be done by the site Construction civil Engineer when the job is going on. There will be a dedicated P & G Safety Officer on site. There will be restrictive access for passage while performing the tasks except those that are working on the project.

ADDITIONAL CONSIDERATION 1. There will be a housekeeping programme to clear the site before and after work is completed 2. The following basic PPEs must be used by the contractor while doing the job; safety Rain shoes, helmet, hand gloves safety goggles etc 3. The area of work will be properly barricaded with caution tape 4. P & G approved disciplinary action/ matrix will be implemented on any personnel/contractor found violating safety rules 5. Any life threatening behavior will lead to an immediate dismissal of the affected individual from site
