IMPORTANT QUALIFICATION INFORMATION FOR PHYSICAL THERAPIST ASSISTANT PROGRAM


It is the student's responsibility to notify Rhodes State College of any name, address or phone number changes. Students
are notified of their acceptance into the program by U.S. Mail. Once offered an acceptance, the student has a specific period
of time to respond. If, for any reason, an invited student does not respond, that student is removed from the qualification list,
and the seat is offered to the next student. The most common reason this happens is because the student failed to update
their address and phone number. To update information, please contact the Advising Office, 148 Public Services Building or
call 419-995-8000.

After qualifying, a student must maintain a 2.5 or higher GPA in order to remain on the qualified list. If the student's GPA
drops below 2.5. the student will be removed from the list. To re-qualify, the student must raise the GPA to or above 2.5.
The student will then be added to the qualification list based on the re-qualification date, which would be the last day of the
quarter during which the GPA was raised to 2.5 or higher.
