Circumstance

This is the record of which a near miss incident was happened on 12 December 2012 at about 10:30am. In the cause of trial shifting of a water tank, the anchor eye welded on the water tank for fixing of pump discharge pipe was accidentally pulled out. The water tank was remained stationary on the platform; no property damage, personal injuries or public nuisance was caused.

Cause of near miss

After deep investigation and analysis of the case by the Site Agent, Assistant Contruction Manager, Site Engineers, Safety Officer and Assistant Safety Officer, the probable causes could be summarised as follows: 3.1 The worker misunderstood the purpose of the anchor eye, which was not for lifting purpose. No suitable identification sign or warning notice displayed to

4 Proposed Improvement Measures The proposed measures listed below should be followed: 4.1 All lifting operation should be monitored and supervised by an experienced foreman/supervisor To prevent any misunderstanding or misusing, all unnecessary anchor eyes should be removed from the water tank. Display warning notice This eye is prohibited for lifting in the vicinity of the anchor eye to warn the workers against misusing. 4.4 Share the lesson learnt from the finding of the near miss incident and clearly explain the particular purpose of the anchor eye to arouse the safety awareness among the workers.
