PROVISION OF MATERIALS, FABRICATION AND OFFSHORE INSTALLATION FOR RBA VENT BOOM WALKWAY, STORAGE CONTAINER AND MISCELLANEOUS ITEMS BLOCK 01 & 02 OFFSHORE VIETNAM
I. SCOPE OF WORK: Description of work is including but not limited to the followings: Contractors shall provide the materials, manpower, tools, equipment and consumable to carry out onshore fabrication for RBA Vent boom walkway, storage container and Miscellaneous items including all consumables for new installation as described in Appendix No. 1. All these structure members shall be supplied and fabricated in accordance with code, standard and specification stated in Appendix No. 1 and its drawings attached. Contractors shall provide manpower, tool, equipments and consumable for removal and offshore installation work of RBA Vent boom walkway, Storage container and miscellaneous items. Contractors shall submit required Quality Assurance documents as stated in Appendix No.1 upon award. Contractors shall submit the Final report together with certificate, documentation as stated in Appendix No. 1 after job completion, upon delivery. Contractor shall submit 02 sets of Equipment dossiers for RBA Vent boom walkway, Storage container comply with PCVL standard practice. MANPOWER, EQUIPMENTS AND CONSUMABLES

Appendix No.1 is an integral part of this scope of work. II. CONTRACTOR, REQUIREMENT:

Contractor shall be experience in provision of steel structure fabrication and offshore installation with the project value from USD 50,000 and above. List of projects involved shall be submitted and endorsed by company person in charge. PETRONAS Offshore Safety Passport is required for all. Contractor is requested to submit documents for OSP application within 05 working days after the date of signing SO in case not holding PETRONAS OSP. T-BOSIET is a must for all OSP applicants. Contractor personnel including valid professional certificates of welders, NDT inspectors must be submitted. List of manpower must be same format as table 01 (for offshore installation) herein below. List of tool, equipments and consumable for offshore installation shall be submitted separately to PCVL for approval during bidding period. List of tool, equipments must be same format as table 02 (for offshore installation) herein below. The valid certificates of equipments and lifting gear shall be submitted to PCVL prior to mobilize for offshore installation. Any other tools, equipments used for offshore installation which not show in this list shall be specified by Vendor. Consumable to be used for fabrication and offshore installation must be complied with required code, standard and specification stated in Appendix No.1

III.

SCHEDULE OF FABRICATION/ PROVISION OF MATERIAL AND OFFSHORE REMOVAL/INSTALLATION: The delivery of RBA Vent Boom walkway, Storage container & Miscellaneous items shall be within 60 days after receiving Purchase order The duration for offshore removal/installation of RBA Vent Boom walkway, storage container and miscellaneous items shall be within 20 days approximately.
