NOTICE OF PUBLIC HEARING

On the ENVIRONMENTAL IMPACT ASSESSMENT (EIA) REPORT of the __________________________________________________ PROJECT of the Proponent _______________________________ to be located in __________________________________________: Notice is hereby given to all parties who wish to give their opinion regarding the implementation of the proposed _____________________________________ PROJECT to attend a Public Hearing on _____________ (Month, Day, Year) at _______ (time to start) at the _____________________________ (exact venue) in Barangay ______________, Municipality/City of _______________, Province of _________________. This Public Hearing is being conducted in connection with the review of the EIA Report of the aforementioned project by the Environmental Management Bureau (EMB) of the Department of Environment and Natural Resources (DENR). All interested parties who wish to attend or participate in this Public Hearing should preferably register in person or by mail with the EMB at DENR Compound, Visayas Avenue, Diliman, Quezon City. Those who will not be able to register or submit written positions may be given the opportunity to share their issues on the day of the hearing itself. However, priority shall be given to those who register first with the above offices before the hearing, and then to the early registrants during the hearing. Full copy/ies of the EIA Report can be accessed at the EMB Region ___ Office and at the libraries/development council office of the Municipal Hall of ___ (state host LGU), while copy/ies of the Executive Summary are available at Barangay/s _________________ (state name of host barangay/s). A limited number of Project Fact sheets are available at these offices. NOTE TO THE PROPONENT: Above Notice of Public Hearing shall observe the following publication dates: Two consecutive publications a week apart, with the second at least 15 days prior to the exact date of hearing, e.g. May 4 and May 11, 2007, with hearing on May 26, 2007.
