Statement:

I started working in the year 2008 for LSA. I receive calls every day. They vary from 10 minutes to an hour per call. The max is 90 minutes. I speak Hindi, Tamil, Malawalam languages spoken in India. I work anywhere from 120 minutes a day more than 2 hours a day Monday to Friday and if there is an emergency they may call me on the weekends. I never worked more than 40 hours. Maybe 15-20 hours a week. I am paid from 2008-2011 it was $.45/min and then 2011-2013 it is $.50/min. I requested a pay raise and they only gave me $.5 more cents. I asked them for $.55 cents per minute and they said it was not possible. I am paid by 1099. LSA does not employer provide insurance of any kind, social security, unemployment, med, workers' compensation or liability insurance. I don't provide liability insurance. I don't advertise for myself or for LSA. I can work for other companies. I do interpretations over the phone only, no face to face. There is no schedules work; LSA just calls me any time. LSA takes the client call, and according to the language then the operator transfers the call to me. I do know the clients like Immigration, detention center calls, insurance companies, U.S. dept of labor unemployment compensation. I spend about 20 minutes per weekly benefit calls, detention center calls can be 25 minutes each. LSA has it setup online where I can see my calls how long they were and how much I get paid. Every 14 days I get paid. I will be able to see 4/1/13 to 4/15/13. I will see it on the call history; I will be able to view it on the 21st of the same month. The records are accurate. I get paid this month for last month. 3/16-3/31 I get paid 4/15/13. The check says name, the amount of the check and how many calls. I work from home. I decide where I work. LSA gives me guidelines. They did a test so that I can take government calls, whatever the government requirements are, I am certified to take the medical calls, they gave me a test and I passed. The tests are online and I took it 2009. They send me paperwork for anything new. They give me scripts to follow, what I have to say to end the call with, "is there anything else that I can assist you with." If I am not available I don't have to take calls. There are no repercussions for refusing work per say. If I don't work, I won't get paid. I don't get paid extra for taking after hour calls. I get paid the same rate per minute no matter who I talk to not even if it is for government calls. I don't get any additional benefits or fringes. I can't hire anyone under me to answer LSA calls. LSA does interpretation and translation work only. I am required to have a landline with no interruptions no internet phones. My phone costs me about $30/month. I supply my phone. LSA doesn't provide any materials or supplies. I am not reimbursed for any purchases or materials, or supplies. I have a written contract that I signed with the company when I was hired, I have been with LSA since 2008. I don't have any opportunity for profit or loss, I am not setup as my own businessi have not made any investment such as insurance or bonding. I don't advertise independently. I don't have a separate business site or letterhead. I have a specialized skill in the three languages that I speak. I could have found work without LSA. I don't know if there are any minors working there. I have read the above and it is correct. X_____________________________
