Situation: Nurse-patient interaction with Dementia/Alzeimer's Disease NURSE Verbal Good morning Mr. Eddie. Non-verbal Still standing by the tree, 2m away from the client. Establishing eye contact while aproaching in a friendly manner. Verbal Good morning. CLIENT Non-verbal Looking at me directly. ANALYSIS I used the technique giving recognition. In here, I greeted the client who was my client again yesterday. This is to indicate that I am aware of his prescence. I established eye contact from a 2m distance. In this, I made sure that I maintained adequate distance from an newly known person. I aproached him in a friendly manner way to establish rapport. As the client respond and I felt that he is interested to talk, I walk closer to him and start the conversation, and again make sure that I maintained adequate distance for normal conversation. Giving information was used as I repeated introducing myself for him to somehow recognize me. Also, the case of my client is Alzeimer's disease, he might forgot of me already and would perceive me as an enemy. The technique used is offering self. I offered myself to sit by his side. And there, is begin the conversation with a broad question. This was done to lead the client in the interaction and it may stimulate him to take the initiative. As asked what brought him here for it to verify if he is oriented of where he is at present. He answers the question but looks around the park. This nonverbal communication is omewhat presents that something bothers or he is thinking deeply of something. As of the moment, I didn't elicit about his non-verbal communication. What I did first was to clarify of what really brought him here. He stated a wrong information which is an indication that he is not oriented of where he really is. And so, seeking clarification is used to clear things out.. And at the same time, he was thinking of something deeply (as manifested by looking around the park) he might not understood the question or confused so I elicited such.

This is an institution for client's with mental problems. Where do you stay then?

Oh no. I mean, I'm a client here I often forgot things. I have this Alze disease that's why.

Yes sir. It's called Alzeimer's disease. You're a client in this institution. You often forget things so this institution is taking care of you for you to somehow relief this disease. I noticed you keep on starring at the plant sir, what are you thinking of sir?

Listening eagerly and afterwards, he again stares at that specific plant

And if he is not really aware of his disease, he will be informed. I may sound non therapeutic to my term "mental problems" but I stated this to present the reality to the client. I had a shocking face(open mouth) at first but closed it afterwards because I remembered the fact that my client has Alzeimer's who forgot things often. Restating and paraphrasing was done for us to know that he communicated the idea effectively. I maintained eye contact for me to express that i am saying the truth and for him to understand very well. As a result, he listens eagerly. The client keeps of starring at the plant. So as for me to clear this curiousity, I used the technique making observation and asked him what he is thinking of. I pointed to the plant to specify the thing I am really refering to. Often, we smile wherenever we remember of the good things happen to us. The client did not smile. As a result, I helped him out to recall the thing that bothers him and for him to be comfortable.

Yeah. This institution has been taking care of me for years now and several people come over to visit me. Just like yesterday, I was so happy because there is this girl who came again and gave me a flower. Same flower as he gave

I'm giving general leads to help him recall of that specific girl. Also, clarifying time and sequence was done to verify if he is putting events in proper sequence. It helps us both (nurse and client) to see them in perspective.

the client became superficial of the topic. He is very interested because this is him main concern of the moment. Exploring was used because it can help him examine fully or recall clearly. Since Alzeimer's clients has marked by deteriorated cognitive fuctioning. Showing pictures could help him recall of the things. Presenting reality was done because true happenings are needed to be presented to recall things fast. The daisy flower was handed to me by Mr. Eddie was a manifesteation that he trust me. He also termed "daughter" in calling me maybe because he perceive me like his daughter. For him not to be confused, I gave the picture to

Returned the flower, Smiling then touched him arms

Yeah. now I remember why I receive daisy flowers often. I'll be waiting for the day my daughter will return. Goodbye nurse.

Smiling so widely, sits, and keep on starring at the picture.

the client for him to differentiate his daughter and me. It was so nice that the client expresses what he feels. He even hugged me as manifestation that he really misses his daughter. At the end, he was able to differentiate me from his daughter. Using of touch (touchiing his arms) was done as an action for thank you
