THE FOLLOWING ATTACHMENTS MUST BE SUBMITTED WITH THIS APPLICATION: 1 A completed copy of the attached application form. 2 A brief description of the funding request, the requested amount and how your organizations programs address the Target Foundation focus area(s). 3 A brief description of expected outcomes or goals during the proposed funding period. 4 A brief description of the organizations history and mission statement. 5 Previous years major accomplishments. 6 A list of the Board of Directors, including names, titles and affiliations. 7 A copy of the organizations IRS 501(c)(3) letter. 8 A current annual operating budget with expenses and income (earned and contributed). 9 A project budget detailing expenses and anticipated income sources. 10 A current list of business and foundation donors and their contribution level. 11 A copy of the organizations most recent audited financial statement. If no audit is available, explain why.
