All of these services take place in the school. No services are participated in outside of the school. Over the past school year (2nd grade), he has missed 8 days of school. Last year in first grade he missed about the same amount if not a little more. Many of his absences are due to health concerns. This students mother works at the University of Illinois with residential housing. The father does not remain active in the students dayto-day life so his place of employment is undetermined. According to this students Emergency Medical Authorization, his health concerns include: Downs Syndrome, trouble swallowing, and a habitual tendency to escape (the classroom). This student also has seasonal allergies. His preferred physician is Dr. Tripathy and preferred hospital is Carle Hospital.

Overall, most of the required information is located in this students school records file. However, his IEP would contain much more in depth information/details about the students personal and
