On this 12th day of May 1855 personally appeared before me, a justice of the
peace within and for the county and state aforesaid Jabish Lewellen aged sixty-
one years a resident of Monroe County State of Missouri who being duly sworn
according to law declares that he is the identical Jabish Lewellen who was a
private in the company commanded by Captain Ganaway In the 13th regiment
of drafted militia commanded by Colonel _laughter at the battle of Orleans in
the war with Great Britain declared by the United States on the 18th day of June
1812 for the terms of six months and continued in actual service in said war for
14 days that he has heretofore made application for bounty lands under the act
of September 28th 1850 and received a land warrant No. forgotten for 80 acres
which he has since legally disposed of and ________.

He makes this declaration for the purpose of obtaining the additional bounty
land to which he may be entitled under the act approved the __ of March 18__.
He also declares that _______ applied for _____ under this or any other act of
Congress any bounty land warrant except the ones above mentioned Jabish
Lewellen.

We James Dale, and Joseph Edwards residents of Monroe County in the state of
Missouri upon our oaths declare that the foregoing declaration was signed and
acknowledged by Jabish Lewellen, in our presence and that we believe from the
appearance of the applicant, that he is the identical person he represents himself
to be.

County of Monroe

On this twenty first day of January AD one thousand eight hundred and fifty
one personally appeared before me the undersigned a justice of the peace within
and for the county and state aforesaid Jabiz Lewellan aged 58 years, a resident of
Monroe County in the State of Missouri, who being duly sworn according to
law declares that he is the identical Jabiz Lewallan who was a private in the
company commanded by Captain William Gannaway in the Regiment ___
Regiment of Kentucky detached Militia commanded by Colonel Presley Gray in
the War with great Britain declared by the United States on the 18th day of June
1812 – that he was drafted at Hartford in Ohio County, Kentucky in the State of
Kentucky on or about the fifteenth day of November 1814 for the term of six
months and continued in actual service in said war for the term of six months
and was honorably discharged at Hartford, Ohio County, Kentucky in the state
of Kentucky on or about the first day of May AD 1815 as will appear by the
muster rolls of said company as the said Jabez Lewellen cannot present herewith
his written certificate of discharge although he ____ he received the same - __
but at this time he paid off for his said services he delivered up his discharge
which has never been mentioned to him. He makes this declaration for the
purpose of obtaining the bounty land to which he may be entitled under the
act granting Bounty land to certain officers and soldiers who have been engaged
in the military service of the United States passed September 28th 1830 – Jabiz
Lewellan

Sworn to and subscribed before me the day and year above written and I hereby
certify that I believe the said Jabiz Lewellan to be the identical man who served
as aforesaid and the identical man who served as aforesaid and that he is of the
age above stated.

On this first day of May AD 1871 personally appeared before me W. Worth

McCarin a notary public within and for the county and state aforesaid Mrs.
Sarah P. Lewellen aged sixty-six years a resident of Monroe County in the state
of Missouri who being duly sworn according to law declares that she is the
widow of Jabez Lewellen, deceased who was a private in the company
commanded by Captain ___ in the regt of Kentucky volunteers under the
command of Major Johnston in the war with Great Britain declared by the
United States on the 18th day of June 1812, that her said husband volunteered at
Hartford in Ohio County, Kentucky on the __ day of ___ 1814 for the term of
six months and continued in actual service in said war for the term of ___ and
was honorably discharged at New Orleans, LA on the __ day of February 1815.

She further states that she was married to the said Jabez Lewellen in Ohio
County, Kentucky on the 5th day of October 1826 by one ____ Hay a minister
of the gospel and that her name before said marriage was Sarah P. Taylor that
her said husband died at Florida, Missouri on the 15th day of November 1868
and that she is now a widow. She makes this declaration for the purpose of
obtaining the bounty land which the ___ be entitled under the act approved
March 3, 1855, and she further declares that she has not received or applied for
bounty land under this or any other act of congress . Sarah Lewellen.

We John Hoyle and Thomas Lewellyn residents of Monroe County in the State
of Missouri upon our oaths declare that the foregoing declaration was signed
and acknowledged by Mrs. Sarah P. Lewellen in our presence and that we are
believed from the appearance and statements of the applicant that she is the
identical person she represents herself to be, and is still a widow.
