A. PROJECT CLUSTER: MEDICAL NUTRITIONAL HEALTH APOSTOLATE (MNHA) B. PROJECT NAME: MNHA: BLOOD SERVICES C. PROJECT RATIONALE 1. There is a continuous and increasing need of blood supply due to various sicknesses. And the only available pharmacy for such medical use are the generous and volunteer human donors. 2. The project is a continuous commitment of the University to the objectives of Republic Act 7719 or the National Blood Services Act of 1994 which promotes voluntarily blood donation. 3. This NSTP Apostolate or project is one avenue where students can exercise and share their sense of Mission to Transform and Beyond Borders, and their four core values of COMPETENCE, CREATIVITY, SOCIAL

INVOLVEMENT and CHRISTIAN SPIRIT. Students are expected to apply and employ their knowledge, skills, and attitude to ensure the success of the project. 4. It is hoped that this exposure of the students will help enrich their professional development, and their Louisian Mission to Transform and Beyond Borders. D. PROJECT GOAL/GENERAL OBJECTIVE v To scout for volunteer blood donors/collect blood supply for the emergency use of medical patients and educate everyone on the health benefits of blood donation on the part of the donor. E. TERMINAL PROGRESS i. SPECIFIC OBJECTIVES During the current term, the apostolate should be able to:

a) Scout and bleed at least 3 successful generous volunteer blood donors per apostolate member; b) Profile the blood type/ blood type of all enrolled NSTP 1 and others who are interested; c) Update the blood type data base of NSTP students; d) Scout for needed donors for specific blood types needed by medical patients who ask assistance from the NSTP or who are referred by hospitals to the NSTP. ii. iii. Coordination with partners: Philippine Red Cross Baguio Benguet Chapter Assisting the Philippine Red Cross in Blood typing & Blood Donation Site visitation at Saint Louis University Gonzaga building, General Luna and Saint Louis University Devesse Plaza, Mary Heights Campus, Bakakeng Having more than 150 people voluntary donating blood v. 1. PROPONENTS PROMOTERS/MODERATORS SLU ADMINISTRATION: endorses and approves the project. NSTP Coordinator: takes charge of the over-all facilitation of the sustainability of the apostolate. The NSTP Coordinator shall co- supervise apostolate as the need arises. SUPERVISING INSTRUCTOR: takes charge of the diligent supervision and monitoring of activities of the apostolate to ensure optimum outcomes and results. Also, the appropriate procedures, guidelines and other plans prepared, approved, implemented and followed. IMPLEMENTERS: Assigned students from the School of Nursing (SON), to serve as the technical working group of the project. In close coordination with their assigned supervising instructor, project students will facilitate the project according to approved plans, procedures and guidelines.

To provide donors privilege card to donors To provide blood donation and blood typing medical supplies during the event To lend or provide available literature or information for the consciousness drive. To record, generate, and submit terminal report on the blood typing and blood donation of SLU NSTP as per format of the NSTP Office. To endorse the apostolate.

v PHILIPPINE RED CROSS(PRC) Benguet: vi. To administer campus blood donation and blood typing at the Maryheights Campus (Devesse Bldg Plaza), Bakakeng,Baguio City. To administer referral blood donors and students for blood donation and/or typing at the PRC Benguet chapter office and accomplish referral forms sent therein. To provide donors privilege card to donors To provide blood donation and blood typing medical supplies during the event To lend or provide available literature or information for the consciousness drive. To record, generate, and submit terminal report on the blood typing and blood donation of SLU NSTP as per format of the NSTP Office. To endorse the apostolate.
