DIRECTOR OF PATIENT SERVICES

BASED AT: ST ELIZABETH HOSPICE ________________________________________________________________________ JOB SUMMARY To work within the multi-disciplinary team (MDT), providing physical, psycho-social and spiritual care to patients attending St Elizabeth Hospice Day Service and their families and carers, with occasional In-Patient Unit cover. To participate as a member of the nursing team, carrying out all relevant forms of care in conjunction with other team members. Responsible for the nursing care delivery, participating actively at MDTs and in decision making. To be a key worker for patients who attend Day Care, being responsible for the coordination of care. To contribute to the multi-disciplinary assessment clinic, providing holistic needs assessment for patients and their families. PROFESSIONAL RESPONSIBILITIES 1. To work jointly within the MDT identifying holistic needs of patients and their relatives, within the context of the total care provided, ensuring that their needs are met as far as possible. 2. To act as a key worker, holding case-load patients and their families on Day Care days. To coordinate the planning, goal setting, implementation and evaluation of care for key working case load. 3. To provide nursing cover within procedure-based Nurse Led Clinic, including planning, goal setting, implementation and evaluation of care. 4. To be a member of the MDT providing holistic assessment in outpatients clinics. 5. To provide accurate needs assessment for Day Care patients, planning and prioritising care. 6. To maintain up to date and comprehensive treatment records. 7. To record, evaluate and keep up to date, explicit care plans which act as effective tools for other members of the multi-professional team. 8. To provide specialist palliative care and advice. 9. To work as part of the in-patient nursing team, as necessary. 10. To administer drugs and treatments within Hospice and NMC guidelines. 11. To liaise as appropriate with community district nursing, social services and other care agencies, ensuring appropriate discharge arrangements are made. 12. To contribute to the provision of therapeutic activity in Day Care in collaboration with the Senior Therapy Assistant and activity volunteers. 13. To promote good practice in palliative care in the community through professional networking and initiating specialist training programmes where appropriate. 14. Together with Day Unit colleagues, identify areas of development within Day Unit and be involved in agreed changes.

MANAGEMENT RESPONSIBILITIES 1. To attend relevant meetings as required.

2. To participate in the development of clinical standards and clinical audit, working within the quality framework of the Hospice. 3. To collect and provide statistical information as required by the Hospice. 4. To co-ordinate personal annual leave requests with other key workers in Day Care and clinic staff to ensure that the unit is fully staffed. 5. To act as a link person for out-patient transport requests, assessing suitability of transport with a volunteer driver where appropriate. 6. To act as a link person for volunteers working within Day Services. PERSONAL DEVELOPMENT 1. To devise with the Day Unit Manager a personal development plan in line with the Hospice objectives and personal needs. 2. To maintain and increase personal professional skills. PERSONNEL RESPONSIBILITIES 1. To recognise indications of staff stress and to facilitate staff support. 2. To participate in the induction of all new staff, as required. 3. To participate in professional placements and lay person visits to the Hospice, as required. 4. To participate in the recruitment of staff as requested. 5. To undertake staff Annual performance Reviews as allocated. 6. To recognise the value of volunteer contribution and play an active part in their support and development where appropriate EDUCATIONAL RESPONSIBILITIES 1. To participate in the Hospice education programme and promote knowledge of psycho-social issues to other staff. 2. To give feedback from attended education programmes/study days to the multidisciplinary team. 3. To maintain awareness of research and relevant findings and in conjunction with the multi-professional team, work towards these being applied to practice. ORGANISATIONAL RESPONSIBILITIES 1. To work at all times within the policies of the Hospice and act within Care Standards, and NMC guidelines at all times. 2. To ensure effective use of resources. 3. To participate in relevant activities which promote the Hospice as requested. 4. To respect at all times the confidentiality of information covering patients, staff and volunteers. 5. To play an active part in the development and maintenance of good relationships with all those who have business with the Hospice. 6. To promote the Hospice Philosophy of Care towards patients, relatives and visitors. Because of the special nature of the Hospice and its work the postholder may on occasion be asked to undertake other duties to help maintain our high standard of care.
