5 hari SMRS, os mengeluh perut terasa kembung. Os merasa perut terasa penuh dan tegang.

Os belum mengeluh perutnya membesar. Tidak merasa mual dan juga muntah. Masih dapat untuk kentut dan masih bisa buang air besar namun BAB sudah dirasakan semakin sulit, keluar hanya sedikit setiap BAB. BAB dirasakan keras. Perut dirasakan nyeri seperti melilit, dan rasa sakit hilang timbul. Os mengeluh rasa yang tidak enak pada seluruh bagian perut. Os tidak mengeluhkan demam. BAK tidak ada keluhan, nyeri (-), darah (-). Ga boleh pake - /+ 5 Days before commotion, patient said that his/her stomach feels bloating. Patient feel his/her stomach filled up and tense. He/she didnt feel his/her stomach bigger. He/she didnt feel any nausea nor throwing up. He/she still flatus and defecate, but his/her defecate getting harder and only a little of feses that he/she defecate, and also constipate. He/she experienced the abdominal pain and intermittent. Patient experienced the abdominal discomfort in every regio in his/her stomach. He/she didnt feel any fever, and he/she didnt experienced pain nor bloody in his/her urine. 3 hari SMRS, os merasa perut semakin membesar dan semakin terasa penuh. Rasa tidak enak pada seluruh bagian perut masih ada. Os mengeluh sangat mual dan mengalami muntah sebanyak 1x, isi muntahan adalah cairan dan makanan yang di komsumsi, dan kurang lebih sebanyak setengah aqua gelas setiap muntah (kan muntahnya Cuma sekali). Os mengeluh sulit untuk kentut. BAB dirasakan semakin sulit, keluar hanya sedikit, dan dalam seminggu ini BAB hanya 1kali. Os tidak mengeluh demam. BAK tidak ada keluhan dengan warna urin kuning jernih, tidak nyeri dan tidak berdarah. Nafsu makan sangat menurun sejak mengeluh mual dan muntah. 3 days before commotion, his/her stomach getting bigger and getting more filled up. He/she still experience the abdominal discomfort. He/she telling that he /she feel nausea and throwing up for one time, the contents of the puke is water and food that he/she eat and the volume is about half of the glass of aqua. He/she felt that he /she was getting harder to flatus and also for defecate even in one week later obly defecate 1 time. He/she didnt experience any fever. His/her urine is in normal line with a yellow colour, there are no pain nor blood. After having the nausea and puking experience, he/she feel lost of his/her appetite.

1 hari SMRS, os semakin merasa lemas. Perut dirasakan semakin membesar dan tegang. Os mengeluh belum BAB. Os juga mengeluh sulit untuk kentut. Mual masih dirasakan os. Muntah sempat berhenti sejak 4 hari lalu namun berulang kembali sebanyak 1x dengan isi muntahan cairan sebanyak setengah aqua gelas. Nafsu makan sangat menurun. BAK tidak terdapat keluhan. Os juga tidak mengeluh demam. Nyeri di seluruh bagian perut juga masih dirasa. Karena dirasakan keluhan semakin memberat dengan perut terasa membesar dan BAB yang sulit keluar, maka Os memutuskan untuk berobat. 1 day before commotion, patient feel limping. His/her stomach getting bigger and bigger and also more tense. Patient said that he/she not defecate yet and also getting harder for flatus. He/she still experience the nausea. And for the puking experience, it stop 4 days ago but it happens again about 1 time /day, the content is water about half of a glass. He/she experience lost of his/her appetite. There is no complaints about his/her urine. There is no fever. Patient still experience the abdominal pain, and because the abdominal pain is getting severe and the stomach also getting bigger, the defecation getting harder, patient decided to coming to the hospital.
