Mail-In Form and Terms & Conditions for the Nokia Q2 Offer

To receive your Nokia $50 Visa® Prepaid Card,

the following steps must be completed by July 31, 2009:
(Please use black or blue ink and print in ALL CAPITAL LETTERS.)

1. A complete photocopy of your receipt showing your purchase of a Nokia E63, Nokia E75, Nokia 5800, Nokia N85 or Nokia N79 phone
between May 11, 2009 and June 30, 2009.
2. A photocopy of the entire bar-code sticker (1” x 4” sticker with various bar-codes) found on the end of the product box your Nokia
phone came in, OR remove your battery from your phone and photocopy the International Mobile Electronic Identity number (IMEI)
by the bar-code on the back of your phone (not the battery).
3. This completed Mail-In Form.

*If any of the above steps are not completed in their entirety, your submission will not be processed.

Please allow at least 8–10 weeks from receipt of request for delivery of your prepaid card.

Please keep a copy of your submission for your records. Mail-In Form, dated store receipt and bar-code photocopy must be postmarked by
July 31, 2009 and received no later than August 14, 2009. Submissions postmarked after July 31, 2009 or received after August 15, 2009
will not be accepted and are void.

Mail-In Forms, store receipts or bar-codes/IMEIs that are counterfeited, mechanically reproduced or altered in any way are void and will not be accepted. Nokia, Inc., and it’s subsidiaries
and/or affiliates are not responsible for illegible, incomplete, lost, damaged, postage-due, mutilated, misdirected or late submissions. Offer not valid if the Nokia phone is returned.
Non-transferable, no substitutions. Offer good for limited time only, while supplies last. You must be a legal U.S resident, 18 years of age or older, to be eligible for the offer. This offer
