Imbalanced nutrition more than body requirements related to excessive intake in relationship to metabolic need. STO: after 30 minutes of health teaching, patient will be able to understand her condition as well as the effects of pregnancy and the disease process: Able to know ways on how to prevent complications on pregnancy . LTO: after 3-4 days of continuous nursing intervention, the patients condition will be stabilized to a controllable level and prevent complications especially on her baby. Diagnostic: Monitor vital signs especially BP, blood glucose level and input and output and record. Review laboratory results

RATIONALE

EVALUATION
STO: Goal met. The patient was able to understand regarding her condition as well as the effects to her baby and her previous pregnancy.

Serves a baseline data to note for alterations in the patient condition

Obtain records for the height, weight body build, gender and age.

Note for any increase or alterations on blood glucose or protein and urine. Provide comparative baseline and helps determine nutritional needs.

Therapeutic: Provide positive reinforcement and encouragement for efforts regarding proper diet on her condition Provide information regarding specific nutritional needs Enhances commitment to program

LTO: Goal met, patients condition was stabilized and controlled. There were no complications noted.

Obese individual maybe deficient in needed nutrients

Educative: Discuss importance of following proper diet for gestation DM patients as well as therapeutic regimens prescribed the obstetrician. Advised small frequent feelings Instruct to have a regular pre-natal check up Discuss smart snacks ( low fat foods or fruits, veggies and low carbohydrate foods). To prevent progress of complications on her condtion
