DESCRIPCIN El test del informador (TIN) o Informant Questionnaire on Cognitive Decline in the Elderly (IQCODE)1 es un cuestionario cognitivo-funcional de 26 tems,

diseado para el scrining de demencia por Jorm y Jacomb en 1989. Est basado en una entrevista estandarizada de Jorm y Korten2 publicada un ao antes. Con posterioridad disearon una versin corta de 16 tems3. En nuestro pas la versin completa fue validada por Morales y cols4 (S-IQCODE). Teniendo como base el SIQCODE realizarn una versin breve (SS-IQCODE)5 de 17 tems. Se trata de un cuestionario que cumplimenta un familiar o persona allegada al enfermo en su domicilio o en presencia del mdico, tras unas breves instrucciones y en muy poco tiempo (5-10 minutos). Se le pide que recuerde como era hace 10 aos y como ha cambiado en la actualidad, segn los criterios de una escala Liker de 5 puntos. La primera versin consta de 26 preguntas con una puntuacin mxima de 130 puntos. La versin breve recoge 17 cuestiones con un mximo de 85 puntos. INTERPRETACIN Se considera de gran valor para el cribado de deterioro cognitivo. Valora memoria, funcionalidad y capacidad ejecutiva de juicio. Los resultados son independientes de la inteligencia previa del paciente, el nivel de escolarizacin y de la edad1,5-7. En la escala de 26 una puntuacin superior a 85 es indicativa de probable deterioro cognitivo. En la versin breve por encima de 57 puntos se considera probable deterioro, con similar sensibilidad y especificidad que la versin larga. Globalmente podemos considerar7 una sensibilidad del 86% y una especificidad del 92%. Los autores opinan que debe considerarse principalmente como una medida continua (no dicotmica) de deterioro cognitivo, si bien en la validacin espaola se le concede un alto valor diagnstico. Se debe valorar si existen preguntas sin contestar, no quedando claro que hacer en estos casos. Su validez parece superior al MMSE de Folstein tanto en la evaluacin clnica de la demencia como en el screening de la misma, sobre todo en fases iniciales. PROPIEDADES PSICOMTRICAS Fiabilidad: Presenta una buena validez interna en poblacin general ( = 0.95) y una aceptable validez test-retest (r = 0.75)1. Las versiones breves han demostrado poseer similar validez que las formas largas del test3,8,9. Validez: Presenta buenos ndices de correlacin diagnstica con el Mini-Mental State Examination5,10, incluso superndolo en ocasiones como indica Del-Ser y cols9 en un estudio con pacientes que presentan estados incipientes de demencia: Sensibilidad S-QICODE SS-QICODE MMSE 86% 86% 57% Especificidad 92% 91% 84% VPP 54% 50% 29% VPN 91% 90% 81%
