INTRODUCTION

There was a recent incident in BUDP01 which involved Loss of Containment at a piping flange joint on line number 300-P-6143B-0004. The piping flange joint included an insulation gasket and a spectacle blind, as shown in sketch below.

An investigation was conducted on this incident and it was identified that the insulation gasket was damaged possibly during installation. This installation process included swinging the spectacle blind and replacing the gaskets. Correct installation procedure was adapted in this case.

INVESTIGATION RESULTS

The following is the results of the investigation conducted on the failed insulation gasket incident: 1. Klinger do not manufacture Phenolic Resin gaskets for 600# piping and above, as this is not recommended by them. Klinger catalogue states Neoprene faced phenolic resin insulation sets used on Class 150 & 300 pipework. Klinger Singapore issued a letter to BSP that confirmed phenolic resin gaskets shall not be used for piping greater than 150#. 2. The 600# phenolic resin insulation gasket installed was not Klinger manufactured. Instead, these gaskets have been cut to size from phenolic resin sheets by Klinger Singapores licensed agent Khong Lieng Trading Co Pte Ltd. This is applicable not only for the subject insulation gasket involved in this incident, but for all insulation gaskets supplied by Klinger. 3. At no stage of the supply had Klinger advised BSP that phenolic resin gaskets are not recommended for 300# piping and above. The reason for Klinger supplying phenolic resin insulation gaskets is that BSP buying description calls for phenolic resin gaskets and hence, in their desire to comply with BSP requirements, they supplied non-standard insulation gaskets which is not in their manufacturing range. 4. BSP Piping Specification BSP-12-S-102 specifies the use of phenolic resin gaskets up to 600# piping. This specification does not specify insulation gasket for 900# piping and above. However, SCM records show that BSP have procured and installed phenolic resin gaskets for all piping classes even up to 2500# high pressure piping. 5. Refer extract from MESC SPE 85/201 below. Shell Global had prescribed the use of glass reinforced epoxy (GRE) gaskets according to NEMA standard LI-1 back in the year 2002.

CONCLUSION

Based on the findings of the above investigation, the following are to be implemented immediatley as way forward on the use of insulation gaskets in BSP facilities. 1. Insulation gaskets of phenolic resin material shall no longer be used. Glass reinforced epoxy (GRE) gaskets according to NEMA standard LI-1 shall be used. For 150#, 300# and 600# piping, GRE G10 material gaskets, Pikotek PGE or equivalent, shall be used. For 900# piping and above, GRE G10 material with 316SS core gaskets, Pikotek VCE or equivalent, shall be used. SAP buying descriptions have been accordingly revised.
