1.0

PURPOSE

The purpose of this method statement is to define the standard work process for Embankment related activities. The method of statement addresses work operation associated with embankment construction. All necessary setting out, tree cutting, clearing and grubbing will be completed prior to performing this operation. 2.0 SCOPE

5.0

RESPONSIBILITIES 5.1 Earthworks Manager: The overall implementation of this procedure and ensuring that the work method statement supports the construction efforts of the project. 5.2 Earthworks Superintendent: Providing technical support consistent with demands of the embankment schedule. Supervising and coordinating the manpower, material requisitions, technical compliance, and monitoring testing activities associated with excavation activities. Implementing inspections through site foreman. Reporting major defects for evaluation Performing all inspection activities Reporting defects for evaluation and records Maintain inspection records as part of associated packages 5.3 Earthworks Foreman: The management and execution in accordance with the scope of works Work in accordance with Health, Safety and Environmental plans and procedures Supervising labor and operators on site. Participating the daily Start Up and work plan sessions. 5.4 Surveyors: Stake out levels for each cross section. 5.5 Laboratory: Laboratory personnel reporting to Quality Control will be responsible performing tests and laboratory tests and for documenting and reporting the results of these tests. 5.6 Quality Control Manager: Check the works according to the requirements. Checking the works according to the requirements. Coordinating the disposition and approvals thereof for documented non conformances. Identifying the need for and coordinating the preparation of detailed inspection checklists. Determining frequencies of ongoing test and inspections of Earthwork Activities. Ensuring review of applicable laboratory or field test reports for acceptability and recording of applicable results on the appropriate checklist. Developing and maintaining a summary sheet of inspection and test results for each activity and of earthwork activity. 6.0 MACHINERY & EQUIPMENT

The following basic machinery and equipment shall be utilized for the embankment construction activities. 7.0 Bulldozers Graders Excavators Loaders Dump Trucks Water Trucks Vibratory Rollers Light Towers for Night Shift CONSTRUCTION METHOD

7.1 Construction Requirements i. Top Soil containing organic matter where found shall be stripped from the ground surface upon the embankment is to be built and the entire surface within embankment limits shall be broken up by ploughing or scarifying to a minimum depth of 200 mm or as otherwise instructed by the Engineer. This layer of material which will form the foundation to the embankment shall then be compacted as specified for the remainder of the embankment. When an embankment is to be constructed over an area previously occupied by a building basement, irrigation canal, well, a previous excavation or another construction that will not permit use of normal embankment compaction methods, construction shall conform to backfilling requirements Work Method Statement for Embankment Construction Page 5 of 14

specified in Clause 206 of General Specification for Roads until normal compaction procedures can be used. Throughout construction, Makyol Co. Inc. will remain responsible for the stability of the embankments and will replace any section that is damaged or displaced and the formation will be kept shaped and well drained at all items. No material shall be placed in an embankment fill that is not classified in accordance with Clause 203.2 in General Specification for Roads as Suitable. If rock fill and soil fill layers are combined in any embankment, the coarser material shall be used for the lower layers. The top 300 mm of all embankments shall be restricted to AASHTO Class A1 (0) and A2.4 (0) soils (as defined in AASHTO M145) or their equivalent as approved by the Engineer. Material of particle size greater than 75 mm shall be excluded. This restriction shall also apply to areas subject either to flash flood or prolonged inundation, such as bridge sites or wadi crossings. Embankments shall be neatly formed to the lines levels and profiles shown on the Drawings. Batter slopes shall not be steeper than shown, except where temporarily necessary to permit adequate edge compaction before trimming to the final shape. Where fill is to be placed against a natural slope or sloping earthworks face, the existing slope shall first be benched as shown on the Drawings or as instructed. Where an embankment is to be constructed against a side slope of 1 vertical to 3 horizontal or steeper, the existing slope shall be benched as shown on the drawings or instructed by the Engineer. The horizontal dimension of benches shall be greater than half the width of the compacting equipment in use. The horizontal dimension of benches shall be greater than half the width of the compacting equipment in use. If at same location this would result in a vertical dimension at the back of the bench more than 60 cm, the horizontal dimension may be reduced. At sand dune areas the first embankment shall be done after the confinement activity as per the instruction of the Client & the Engineer during site visit on 02.11.2010. This confinement (at OGL) will provide a smooth & stable working area for earthworks machinery & equipment and for the embankment. The confinement sections will be constructed with fill material (wadi materials) on both slope edges in layers (0.20m thick). Dimensions of the confinement lines will be 4.00m width and up to 0.60m height. The excess material will be trimmed at slope cutting activities. After the construction of edge confinement lines, sand filling will be executed up to 0.60m (from OGL to NGL) as it is shown in below figure. Sand layer shall be watered and compacted prior to the construction of 1st layer of embankment.

Embankments shall be raised to subgrade level evenly across their full with from edge to edge. Excess width will not be permitted other than is needed for proper edge compaction and to allow trimming back to the required profile and finishing as shown on the Drawings. Side slope protection shall be as shown on Drawings or instructed by the Engineer. Soil fill shall be placed and spread evenly to a loose-layer thickness not more than 250 mm before compaction. No fresh layer shall be placed or spread until the preceding layer or surface has been compacted as specified and approved. Water shall be added as necessary to maintain optimum moisture content of the fill material. Compaction plant shall be operated so as to give a uniform soil density in each layer. All plant, vehicles and equipment shall be routed evenly over the surface of each layer.

7.2 Compaction Quality Control i. ii. iii. Testing shall be carried out as per AASHTO T-191, the reference density shall be the MDD obtained by AASHTO T-180 (Method D). The moisture content of each fill layer at the time of compaction shall be with in =1.5 % of the optimum moisture content. Layers placed more than 300 mm below embankment subgrade level shall be compacted to 90% of the maximum dry density. For all fill soils containing more than 10 % oversize material retained on a 19 mm sieve, the MDD obtained shall be adjusted for the oversize material as per AASHTO T-224 or by a procedure approved by the Engineer. Layers placed in the top 300 mm of the embankment shall be compacted to 95% of MDD. The Frequency of Quality Control density testing of each embankment layer shall be 1 per 150 m3 of compacted soil or as instructed by the Engineer. The Frequency of Quality Control testing for the specified subgrade material shall be at least 1 test each of sieve analysis and Atterberg Limits per 1500 m3 and at least 1 CBR test per 5000 m3 of the subgrade material or as instructed by the Engineer. Where embankments are placed adjacent to structures or at locations where it is not practicable to use normal plant, each layer shall be compacted by use Work Method Statement for Embankment Construction Page 7 of 14

of pedestrian rollers, mechanical rammers or other suitable equipment. Each layer shall be compacted to the specified density and shall not be greater than 200 mm (loose measurement) in thickness. If necessary and as instructed by the Engineer, Trial embankment sections shall be constructed. The fill soils used in the trials shall be those intendent for the Permanent Works and the compaction equipment used shall be listed in the Programme of Work intended for the relationship between the number of compaction plant passes and fill layer density attained and shall be continued until a satisfactory procedure is established.

7.3 Embankment Construction Execution Plan 1. FILL PREPARATION 1.1. TRIAL STRIP A trial strip may be prepared in order to determine the number and the sequence of the passes to be performed for fill layers. The procedure for the trial strip shall be as follows: 1) The number of passes will be determined in advance on a trial strip of 200 m x 12 m 2) For each pass, three random points on the strip with good surface conditions will be selected and the degree of compaction shall be measured. 3) After each pass of the roller, the compaction tests will be taken in the same exact spots. 4) The results of three spots shall be averaged and a degree of compaction vs. number of passes curve will be plotted. 5) Once it is determined that additional rolling does not increase the degree of compaction, rolling operation will be stopped. 6) The sequence of rollers and the number of passes required to obtain the Target Degree of Compaction as determined from the control strip will remain the same for the remainder of the fill course represented by the control strip. According to the specifications, Target Degree of Compaction shall be not less than % 90. 1.2. MASS GRADING Mass grading is the actual cutting of high points and filling of low points to achieve a relatively smooth fill surface. During mass grading process, traffic of construction machinery and vehicle over the fill area will be utilized as a proof-rolling means to identify local weak spots which reveal themselves as excessive rutting and deformation on the surface. The staking process consists of defining side slope bottom at certain chainages and at certain fill layers. The staking process is as follows: At every 20m the intersection of embankment slope and below layer (either natural ground or previous fill layer) will be defined along the road segment. Number of stakes will be increased at critical road segments such as horizontal curves where more accuracy is required. Fill thickness will be marked on the stakes for every fill layer which is of 25cm. During spreading of fill material, stakes that may have come off will be renewed by a survey team. While compacting, stakes will be cleared from the way of rollers. 2. CONSTRUCTION OF FILL Spreading of Material Construction of fill layers consists of spreading of appropriate noncohesive fill material by layers and compaction to the required level specified by design. Fill material shall be deposited to site by dump trucks and spread either by a dozer or a motor grader in layers not exceeding 25 cm compacted thickness using graders and front wheel loaders. The layers will be graded, leveled, pulverized and mixed with water as necessary. The fill layer will be homogeneous with respect to the amount of water. Compaction of Material Compaction of material will be handled using single drum smooth-pad rollers and/or single drum padded rollers. Compaction direction shall be from lower elevations to higher elevations and passes shall be performed so that drum of the roller will overlap its previous pass by %10. Next set of passes shall not be performed unless a set of passes is completed on all lanes. Rollers shall perform their last passes without vibration so as to attain a smooth finishing surface. If fill material is too dry and having moisture below its optimum content (between %5-%15, to be determined by Standard Proctor Test), water trucks shall be utilized so as to attain optimum water content (between %5-%15, to be determined by Standard Proctor Test). In the case that fill material has a moisture content above optimum, material shall either be ripped and aerated or be waited until excess water Work Method Statement for Embankment Construction Page 9 of 14

self-drains or evaporates down to optimum water content (between %5%15, to be determined by Standard Proctor Test). Once the required number of passes is performed on the entire strip of the fill, the degree of compaction shall be measured at a number of randomly selected points so that the number of test points represents the strip accurately. Provided that target degree of compaction is reached, same procedure will be followed for the next strip of fill. For the adjacent fill strips, material spreading process will be performed so that one fill strip overlaps the adjacent one to assure stability and uniform behavior of fill layers.

8.0

MEASUREMENT METHOD

No direct measurement or payment shall be made for Embankment Construction. Costs incurred are deemed to be included in the Bill Items for Suitable Excavation to Embankment or Borrow Excavation to Embankment. 9.0 HEALTH AND SAFETY

This is accomplished and controlled by deploying our HSE is not an optional extra! concept as the project culture. A key feature of this is our dedication to the philosophy of zero accident which will be linked to the companys Incident and injury free philosophy. This value based approach aligns with Companys HSE perception and MAK-YOL CO. INC. shall apply the same approach to our sub-contractors so that they adopt and implement similar values. By this, all MAK-YOL CO. INC. employees and sub-contractors shall conduct themselves and execute their work in a safe and healthy manner and establish the same approach for the protection of the client property, as well as the natural environment. This standard shall be communicated to all those persons involved in this project, irrespective of nationality or language. Each person on the project is considered a stakeholder in this HSE commitment. The MAK-YOL CO. INC. work scope will be managed in line with MAK-YOL CO. INC.s HSE Management system, which can be viewed as a pyramid that has five layers; Work Method Statement for Embankment Construction Page 10 of 14

The HSE management systems pyramid is built from generic to specific. The foundation, which is fundamental, is Management Commitment. The commitment made is that HSE has equal priority with any other aspect of our business. This is detailed in the policy statement overleaf, which is signed by the MAK-YOL CO. INC. General Manager. The next level is the HSE Manual (procedures) that specifies our safe systems of work. A project specific construction risk assessment and procedures that are tailored to be directly relevant to the work to be performed comprise the third level. The fourth level in the HSE management system pyramid consists of Job Hazard Analyses (JHA) where work-package or task based upfront activities is the basis for daily site activities. Finally, the pinnacle of the pyramid is Innovation. Safety Program will consist at a minimum of the following: Management commitment in writing (refer to General Managers MAK-YOL CO. INC. HSE Policy Statement). Additionally, every two weeks the MAKYOL CO. INC. General Manager and the MAK-YOL CO. INC. Construction Manager write a column in the MAK-YOL CO. INC. Safety Line Newsletter that focuses on key safety issues of the day. Hands-on leadership by visits to field sites and by speaking at scheduled safety meetings on commitment to safety, health and well being of the workers on this Project are also key to the MAK-YOL CO. INC.s management commitment. Remove barriers that cause incidents and accidents. Weekly site meetings with supervisors and workers will be the focus on two-way communication and will highlight potential roadblocks to Zero Accidents and Incident and Injury Free workplace. Positive, reinforced feedback and recognition for the correct behaviors will be highlighted and recognized. Resources (money, human and hardware) will be dedicated and provided to this project in support of the HSE management systems. At no time during the life of this project will cost or schedule concerns supersede HSE priorities. HSE program on the project will be led by Site Manager and supported by Site HSE Manager. Safety in execution will be the number 1 priority on this project. MAK-YOL CO. INC. firmly requires that line managers accept accountability for safety and this philosophy and extends through the project. Strive for continual improvement. MAK-YOL CO. INC. believes that excellent HSE performance should be a core value of any World Class Company. Whilst measuring reactive indicators is an essential element in any HSE Program the main focus proposed by MAK-YOL CO. INC. is on establishing and measuring a range of proactive Key Performance Indicators (KPIs) that will be implemented by the project team, in order to achieve an Incident and Injury Free environment. All personnel will successfully complete all required Company training requirements and will receive an additional two hours of MAK-YOL CO. has already committed resources for training and has obtained approval to conduct in-house training for specific programs. MAK-YOL CO. INC. will continue to dedicate skilled resources to this project enhancement. Extensive WRP (work responsible person) training will be deployed to enhance the quality of the first line site supervision. Work Permit courses will be held in addition to the client training programs to emphasize and enhance the understanding and the value of the use of work permits as an integral part of the project safety program. Safety Task Analysis will be prepared and updated as required and used as basis for daily work talks and in the execution of daily job performance. This is in addition to the construction risk assessment and will be a key focus for leadership on the job site. Each foreman and lead hand will keep on his person a copy of the STA for his assigned tasks. It will be used as an adjunct to the daily toolbox talks. A program of daily observations, weekly and monthly site inspections and regular audits will be implemented with MAK-YOL CO. INC. management representatives and action on any noted discrepancies will be acted upon promptly. Results are published and used as a part of the weekly HSE briefing to inform the workforce of good and poor points. Subcontractors HSE Performance will be reviewed and assessed regularly. Behavioral Based Safety will be employed and will be extensively covered in all training and safety sessions. The focus of this program is to insure that all personnel understand the consequences of satisfactory and unsatisfactory behavior on this project. Safe Work Observations Teams will be initiated within the project work force. Data will be collected and reviewed daily. This program is a further expansion of the Behavioral Based Safety Program. Daily Toolbox Talks and weekly HSE meetings will be a normal part of the project work activity. Weekly HSE inspections results will be used the next morning as the basis for weekly HSE meetings. Each employee will be briefed on the results and the findings will be openly discussed. Any incidents and near misses will be reported promptly and rapidly investigated to determine root cause and to identify any deficiencies in procedures or execution plans. Safe tools and safe equipment will be utilized on this project. All equipment and hand-tools shall be inspected, checked, and recorded. This includes daily safety checklists for all trucks and heavy equipment. Items requiring color-coding will be properly documented. QUALITY CONTROL- according to ITP requirements Earthwork inspection and testing criteria has been established in the reference technical specifications. laboratory tests with testing criteria shall be identified by listing and describing on the Earthwork Inspection and Testing Matrix. The Matrix checklist shall specify the required inspections/tests, provide reference to the test method, and indicate scheduled tests and inspection test result. Inspections identified on the matrix are performed by Quality Control Inspectors with direction provided by the QC Engineer. Laboratory testing shall be performed in accordance with approved laboratory procedures and appropriate standard or code of practice. Tests and inspections shall be performed as outlined on attached partially completed checklists and as directed and/or expanded by the QC Engineer. Tests shall be performed in accordance with the specifications and standards, when necessary by detailed field or laboratory procedures. Additional inspection items of detailed routine earthwork activities performed by construction may be added to the check list at the discretion of the QC Engineer utilizing additional copies. Additional earthwork or related material inspections/tests may be developed by the QC Engineer based on new or revised specification requirements or as directed by the QC Manager. These tests and inspections shall be outlined on checklists prepared and processed in accordance with this procedure. Related laboratory and field tests shall be performed in accordance with existing or specially created, approved laboratory procedures. Quality Control Inspector shall monitor Earthwork activities to assure that both the material and Earthwork methods are acceptable. Subsequent testing and inspection frequencies shall be in accordance with requirements of applicable Standards and Specifications as directed by the QC Engineer. Non-conformance shall be resolved in accordance the specifications. Nonconformance prior to Quality Control acceptance of material and Earthwork activities. Deficient conditions noted during in-process work (i.e. out-of-spec. density tests or lift thickness inspection results) need not be recorded on a formal Non-conformance Report, but the condition shall be corrected and retest/inspection results shall be satisfactory before ongoing work is allowed. The deficient condition and subsequent verification of resolution shall be recorded on the applicable Inspection and Testing Matrix form. The tests or inspection reports for resolution shall be referenced to the failed reports. Inspection shall be recorded and reported using a standard inspection report and inspection checklist (when applicable) to the Site QC Manager. Tests data and results shall be recorded and reported on appropriate test reports to the Site QC Manager. Acceptance or rejection shall be noted on the applicable checklist. The result of the Earthworks Tests shall be available for use by the Client. All Test Records for construction quality control shall include the location of the test (ex. chainage and offset from centerline), elevation and source of material (ex. stockpile excavation or barrow pit). The summary sheets shall be supplied to the Client on a monthly basis and shall be accompanied by a summary of the actual results so that statistical analysis may be carried out.
