30 DAY NOTICE TO VACATE


This Eviction Notice is given to you on this day 21st Day of February, 2013 You are being asked to leave the premises. If you do not leave, court-ordered actions may be initiated against you. If you are in doubt regarding your legal rights and obligations, it is recommended that you seek legal assistance. Please take notice as to the Clauses that were violated in the said lease agreement: 11-1, 11-2, 11-3, 132, 13-3, and 24 The lease agreement states there will be 2 people residing in the said apartment #8. 3 Have been residing there periodically over the coarse of 6 months. Complaints of your personal trash (from the neighbors) have been strewn though-out the parking lot, flower beds, and yards. Excessive noise disturbing the neighbors and property damages to the apartment #8 in excess of $500 - Broken Front Door, Broken Storm Door, Holes in the Walls in the Kitchen, and a severe Roach infestation. PLEASE TAKE FURTHER NOTICE that within FOURTEEN (14) days after service of this notice, you are hereby required to perform the aforesaid covenant(s) WITHIN FOURTEEN (14) days or vacate the premises within THIRTY (30) days as required by this notice will result in forfeiture of the lease and/or rental agreement and will institute legal proceedings against you to recover rent, damages and possession of the said premises. THIS IS INTENDED AS A THIRTY (30) DAY NOTICE FOR THE PURPOSE OF TERMINATING YOUR TENANCY.
