Health Assessment and Health Promotion


The aim of this paper is to investigate various influences and determinants that may have an consequential impact on an individuals health outcomes throughout their life, the health status of an individual can be influenced by their interactions between others and their environment; this is uniquely to ones self. The paper will be exploring different types of health assessments and frameworks that could be utilised in the nursing profession. Firstly the paper will be exploring an individuals health, utilising the four basic Nursing Models outlined by Callista Roy in Nursing Process; Application of conceptual models (Paula J. Christensen & Kenny, 1995). Then the paper will be utilising an assessment tool with the focused health assessment as well as analysing the data collected from the assessment tool used. There is a research critique on the research for a specific health issue, which leads into an article critique. The paper finishes with the short essay on the health issue stress, the short essay will briefly go over stress interventions and how the nurse can help to reduce the stress responses and triggers in individuals.

Focused Assessment
Throughout the self-health assessment there are some aspects that were flagged and could possibly be investigated in more detail. The discovered aspects were the psychological concerns. It was uncovered that Kelly Wilson was not getting much sleep throughout the night as he was only getting four (4) to six (6) hours sleep each night and expressed a low self confidence about his physical self and finds it hard to take on new changes; new surroundings and new people. For the focused health assessment the chosen health issue or potential health issue that will be the centre of the assessment will be depression, anxiety and stress. A tool have been selected to assist in collecting data from the client and give an overview in analysing the collected data, the tool that will be utilised is the Depression, Anxiety and Stress Scale (known as DASS 21). The DASS 21 is an estimable measure according to quantity of distress along the three axes of depression, anxiety and stress, it does this by isolating and identifying aspects of emotional disturbances of an individual, by which giving a health professional a look-in of the mental status of a person as well as a rough idea where to focus their care needs. Analysis of the data collected Looking at the results of the DASS 21 and comparing the scores to the range value indictor, it is evident that there are some hidden issues and health concerns; Kelly has some depression with a score of 6, moderate anxiety level with a score of 6, however his stress results has shown possible heightened issues; scoring 15. The results are indicative of possible high stress levels accompanied with moderate anxiety. These scores will help the nurse or clinician in conjunction with developing care strategies that may assist the client to reduce or manage possible problems that is relevant to the outlined health issue, the results can also highlight the area that may be of a contributing factor to restless sleep at night such as stress or anxiety.

Research Critique The websites utilised to research the health issue, and to help improve the condition are a combination of independent health organisation, government health organisation and peer reviewed supported specialist of their field. Better Health Channel Victoria is a website that provides health and medical information which is quality assured, reliable, up to date, easy to understand, regularly reviewed and locally relevant; it is an initiative of the State Government of Victoria. Anxiety Treatment Australia is a website that provides information on the anxiety disorders, treatment options, psychologists who can help and treat anxiety, group therapy and workshops, support groups, articles, as well as resources and links. This website is a hub to credible information as stated in its own disclaimer highlighted on the website. The websites all gave relevant and insightful information in regards to the health issue. The sources of information contained within the websites were current as shown by their comprehensive and recent reference lists with the sources cited in-text to help back-up the articles main idea to explore the condition and its varied influences that may have an impact on an individuals health as well as treatments. The information contained in the websites were built on objectively, and was backed-up by a current research base with all evidence acknowledged and referenced.

Annotated Bibliography Mindfulness is a technique that is used for therapeutic therapy or as part of religious practice; this technique can be used for mental health problems or a way to destress oneself. The article concentrates its attention on mindfulness technique and its application of reacting to or relieving the symptoms of stress. Mindfulness can be achieved when you start to pay attention to your surroundings and stop doing multiple things at once as it is this connection with the present that many people loose site of, mindfulness teaches people to live each moment as it unfolds. The article continues on providing many aspects of the technique, which helps to adopt and embrace mindfulness.

Health Promotion Essay


Stress can be described as an event in which environmental or internal demands (or both) tax or exceed the adaptive resources of the person (Moore-Ede, Czeisler, & Richardson, 1983) Nurses can help individuals to analyse and manage with; the stressors in their life, nurses can plan the nursing interventions to help individuals to manage their stress responses as well as stress related illnesses. This short essay will briefly go over stress interventions and how the nurse can help to reduce the stress responses and triggers in individuals. Stress can take a heavy tax on individuals as they can experience trouble focusing on instructions, staying on task, or problem solving as well as personal and physical difficulties that are associated with stress. When stress affects an individual they use both physical and emotional energy to react and adapt to the stressor; the defence mechanisms may be used as well. The nurse needs to work collaboratively with the individual to help fortify capableness of coping with stress, to do this the successfully the nurse can utilise specific adaptive therapeutic approaches such as education, cognitive be behaviour therapy, and assertiveness training. Education is a crucial component of nursing care, it aims to utilise education to help improve the clients adaptive response to stress (Funnel, 2005, p. 153). The nurse collaborates with the individual client to analyse what education that can be of use in the particular situation such education and education information may be designed around the stressors that precipitate anxiety, coping resources and adaptive resources. Goals may incorporate helping the person to recognize the signs and symptoms of their stress and gain knowledge of coping mechanisms or cognitive behaviour therapy, such technique may be self-training which incorporates and promotes positive self talk and positive thoughts, for example I am ok, I can manage, I can do this. Self-training helps people regain control of a situation or when stress start to hit when things are becoming too overwhelming.

Mindfulness teaches people to live each moment as it unfolds("Mind over matter," 2011), and is a type of cognitive therapy. The nurse can utilise this technique as a powerful therapeutic tool as it had been found mindfulness techniques can help prevent alleviate anxiety and reduce physical symptoms... of stress. The nurse can direct and instruct the individual on how to use the mindfulness technique; the steps are simple and easy to follow as they can be done anywhere, where it is quite. Stress can occur and hang around because some people cope with stress and react to stress differently passive (submissive) or aggressively (Funnel, 2005, p. 154). Assertiveness training can help individuals learn to be assertive in turn helps them to be in control of situations that effects their lives; the sense of control can reduce the stress. Assertive behaviour is a communication that is open and honest of the individuals personal views, needs and emotions this is neither passive or aggressive, the nurse can facilitate the training of assertiveness behaviour over several sessions to help the individual to learn the technique properly. Wilma Kirsten says in her article Stress: Symptoms, Causes, Approaches for Stress Reduction relaxation techniques such as deep breathing and ... exercise which can include air sprint or a brisk walk (Kirsten, 2012, p. 3) can help to reduce stress as this can be done anywhere and anytime, for example if you on lunch go for a walk away from the workplace this will help you be refreshed when you come back. The nurse can educate the individual the benefits in choosing to have a well-balanced meal, which includes protein such as chicken or hummus avoid sugary foods (Kirsten, 2012, p. 3). Eating a healthy diet can help give you long lasting energy which will help you get through the day without feeling tiered or fatigued. Through specific adaptive therapeutic approaches such as education, cognitive behaviour therapy, and assertiveness training as well as dietary support the nurse can help the individual to overcome or manage their stress before it becomes too overwhelming.
