That sometime on February 22, 2013, Friday at around 2:00 to 3:00 o clock in the afternoon, as I was on my way to deliver and pay the check to the payee, D3 Sisters J.S, the check was lost. That despite my diligent efforts exerted, I could no longer locate, find or recover the said aforementioned check and to the best of my knowledge, the same is truly lost. The said check has not been confiscated by any officer of the law or the issuing office or agency due to the violation of any law, statute, order, rule or regulation. In the event that the lost check should hereafter be found or located, I undertake to forward or surrender the same to Office of the Municipal Treasurer for appropriate disposal. That on December 18, 2012, in lieu of me, Ms. Quennie Diane S. Cordero, 23 and a resident of Brgy. Licod Tanauan, Leyte brought the said unit of phone to Digital Gleen to be repaired for it was still under warranty for one (1) year having purchased and applied a plan under Globe Telecom. That she was looking for the copy of the job order receipt of my cell phone but to her surprise, said receipt could not be found; That she took pains to look for the aforesaid job order receipt, but to no avail; That I am executing this affidavit to attest to the truth of the foregoing and to request from whomever is in-charge in the releasing of the said cell phone unit.
