Applying a Condom Catheter

Procedure Guidelines 1. Perform hand hygiene. Apply gloves. 2. Provide privacy by closing the bedside curtain or door to the room. 3. Place bed at a comfortable working height. 4. Help the patient into a supine position and place a bath blanket over the upper torso. Fold down the top linens so that the legs are covered and only the genitalia are exposed. 5. Prepare the urine collection bag and tubing. A. Clamp the collection bag port and secure the bag to the bed frame. B. Bring the tubing up between the side rail and bed frame. C. Make sure the tubing wont be pulled when the side rail is raised. D. As an alternative, prepare a leg bag for connection to the condom catheter. 6. Provide perineal care and dry area thoroughly. If the patient is uncircumcised, return the foreskin to the normal position. 7. Apply the skin preparation to the shaft of the penis (but not the glans) and let it dry. If needed, clip or shave the hair at the base of the penis to prevent pulling and discomfort. 8. If the condom catheter includes an adhesive strip, apply this next. (Not all brands of condom catheters include an adhesive strip. Some brands require that you apply the condom catheter first and then spiral wrap the penile shaft with a strip of elastic adhesive.) Make sure the strip is not too tight. Be careful not to overlap the strip on itself.

Procedure Guidelines 9. Using your nondominant hand, grasp the penis along the shaft. Using the other hand, smoothly roll the condom sheath onto the penis, leaving 1 to 2 inches between the tip of the glans and the end of the condom catheter. 10. Connect one end of the drainage tubing to the condom catheter, making sure that the condom is not twisted. If using a self-adhesive catheter, apply gentle pressure on the penile shaft for 10 to 15 seconds to secure the catheter. If using a leg bag, connect one

end of the tubing to the condom catheter and the other end to the leg bag. Then attach the bag to the patient with the leg straps. 11. Coil the excess tubing and secure it to the bottom sheet on the bed. 12. Lower the bed and replace the side rail for safety. 13. Discard gloves and perform hand hygiene. 14. As part of follow-up care, assess the penis 30 minutes after condom catheter application. Note any swelling, discoloration, or discomfort. Reassess the penis at least once a day when performing hygiene or changing the condom catheter.
