COMPANY NAME will ensure compliance with applicable laws and re ulations, ta!e prompt action to correct unsafe conditions, and continued safety education for all concerned. Protection of employees from in$ury or occupational disease is a ma$or continuin ob$ective. All employees are responsible for followin safe wor! practices and must be dedicated to the continuin ob$ective of preventin harm to themselves or others and dama e to assets.

ENVIRONMENTAL -OLICY
1s an 9#: 1/((1 registered company !:3P1N; N13' re+uires all contractors while on$site to conduct their activities and operations with respect to the environment so that any impact on the environment is minimi5ed. 9t is the contractor<s responsi"ility to ensure all of their employees read and understand !:3P1N; N13'<s 'nvironmental Policy.

)(. /ENERAL SAFETY RULES


1. 1ll work must "e performed in a safe manner as dictated "y the #afety Regulations for !onstruction Pro=ects and the :ccupational &ealth and #afety 1ct and any other applica"le regulations. 1ll materials and processes will meet all applica"le codes and regulations. *. #ufficient precautions must "e e>ercised to protect workers and property from any ha5ards that may "e associated with the work "eing performed ,i.e. "arricades caution tape ver"al communication etc... !ontractors and their employees are re+uired to report any incidents?accidents damages or use of fire e>tinguishers immediately to their supervisor and to the !:3P1N; N13' !ontact 'mployee. 1n accident report must "e completed and a copy provided to !:3P1N; N13'<s @&#! within */ hours of the incident. !ontractors at their own e>pense will "e responsi"le to repair damage to e+uipment and "uildings caused "y careless and?or unsafe acts "y their employees. /. No food or "everages are allowed in the production areas of the plant. 0. %he property is off$limits when your work is over. 2. %he speed limit on company property is 1( km?h. 4. Park your vehicle in the areas designated "y !:3P1N; N13'. 6. )asoline and diesel powered vehicles are prohi"ited inside the facility unless approval is o"tained from the !ontact Person. 8. 1ll contractors working at !:3P1N; N13' must have full 7#9A coverage for their employees as well as lia"ility insurance coverage for at least Five million dollars. Proof of the a"ove must "e su"mitted to !:3P1N; N13' annually or upon re+uest. +(. -ERSONAL -ROTECTIVE E0UI-MENT 1--E2 1. #afety glasses are mandatory within all !:3P1N; N13' facilities. 1nyone not wearing approved safety prescription glasses shall wear standard safety glasses with attached side shields over their glasses. *. 1ll safety glasses must "e certified to !#1 standards which include attached side shields. .. #teel$toed safety shoes?"oots must "e worn in all designated areas of the facility. /. 'ar protection must "e worn in all designated areas of the facility. 0. Bevlar sleeves are mandatory in all parts of the Arampton and 3ilton facilities and are availa"le at reception or from the !ontact 'mployee. 3(. EVACUATION -ROCE,URE 1. 1ll contractor employees must sign in and out at the "eginning and end of each shift. %he Cisitor ? !ontractor<s #ign$in Aook is located in the reception area.

*. 9f the alarm siren sounds contractors must shut down the e+uipment they are working on where safe to do so and e>it the "uilding through the nearest e>it as +uickly as possi"le. .. :nce outside contractors will proceed to the area noted "elow and await further instructions. Plant 1 location D the circle in front of the "uilding Plant * location D the grassy area =ust "eyond the parking lot Plant . location D the area "etween the weigh scale and fence in the west part of the parking lot /. %he !ontact 'mployee will ensure that all contractors they are responsi"le for are accounted for. 0. :nly re$enter the facility when the E1LL !L'1R< has "een given. 2. 1ll contractors and employees must familiari5e themselves with these procedures including the location of e>its and fire e>tinguishers. 4(. SAFETY LOC5OUT 1. %he !:3P1N; N13' Lockout Policy will "e o"served at all times. Lockout Policy (Zero Energy State) Prior to any wor! bein performed on a process or e&uipment, the process or e&uipment must be isolated to a 'ero Ener y (tate. All possible sources of ener y )electrical, mechanical, hydraulic, pneumatic, fluids, ases and ravity or other forms of potential ener y* which needs to be isolated must be+ ,dentified ,solated by openin switches )main disconnects* closin valves, blan!in lines, releasin pressures, removin fuses and . or usin bloc!in to ma!e sure the process will not start accidentally, and /oc!ed such that they cannot be removed. After the process or e&uipment is isolated and loc!ed out, the power sources must be chec!ed to ensure that the process or e&uipment is, in fact, in a #ero ener y state and that the appropriate ener y sources have been loc!ed out. *. 1ny person voiding the intent of the personal safety lock or failing to follow proper lockout procedures will "e su"=ect to disciplinary actions as per !ompany Rules and Regulations as laid out in the 'nforcement section in this document. .. 'very person ,production maintenance staff contractor visitor etc.- involved in the work or inspection on a process or piece of e+uipment must lockout. Provision for multiple locks shall "e made using a multiple lockout clamp. /. %he standard method of isolating e+uipment in a 5ero energy state shall "e a personal safety lock with a tag. !ontractor<s tag shall have the worker<s name and phone num"er and the company name and phone num"er.

Locko%t -roc#&%r# 1. Aefore any work is performed all possi"le sources of energy and ? or motion must "e reduced to a 5ero energy state. *. #afety locks shall "e affi>ed as to prevent the physical movement of the isolating devices ,switch valve lever etc.-. Power sources must "e tried to make sure they have "een isolated. .. 'ach person is re+uired to work on or inspect e+uipment which may present a ha5ard must lockout. /. No person shall work under the protection of any lock other than his ? her own. 0. #afety locks are only to "e removed when the e+uipment is safe operational and all guards are in place. 2. No lock shall "e removed other than "y the person who affi>ed it. 6(. HEALTH AN, SECURITY 1. 1ll contractor employees may not enter areas other than where they are assigned without "eing accompanied "y an !:3P1N; N13' employee. *. !:3P1N; N13' is not responsi"le for loss damage or theft of materials e+uipment tools or personal "elongings .. %he use possession or sale of controlled su"stances alcohol or weapons of any kind on !:3P1N; N13' property is strictly prohi"ited. /. #hould a contractor employee have any special health conditions that we can assist with or should "e aware of please contact the &uman Resources Department for confidential assistance. 0. !:3P1N; N13' facilities are F#moke$FreeG 'nvironments. #moking is allowed only in designated areas that are posted accordingly. #moking is strictly prohi"ited within the "uilding and within 1( meters of compressed gas?propane storage facilities. 7(. CHEMICALS AN, MATERIAL SAFETY ,ATA SHEETS 1MS,S2 1. %he !ontact 'mployee '3# !oordinator or &ealth and #afety !oordinator reserves the right to refuse any product that may endanger !:3P1N; N13' employees or the environment. *. Permission to store materials on !:3P1N; N13' premises must "e o"tained from the !ontact 'mployee in advance. 1ny stored materials must "e properly la"eled and kept in a designated location. .. %he 3#D# for chemicals used on the !:3P1N; N13' premises can "e found in "inders at the location noted "elow. Plant 1 location D :utside of the supervisor<s office and the front office Plant * location D :utside of the +uality la" Plant . location D :n the door of the #tamping and 7elding offices or in the &R Department.

/. 1ll materials "rought onto !:3P1N; N13' property must "e accompanied "y the appropriate 3#D# and presented to !:3P1N; N13' for approval prior to "ringing the material on$site. 0. No chemicals of any kind regardless of +uantity will "e left on !:3P1N; N13' property. 2. 1ll chemical containers will "e removed from !:3P1N; N13' property when work is completed. 4. 1 proper 7&39# la"el must "e affi>ed on all chemical containers and workplace la"els are also re+uired if decanting to another container. 6. 1ll chemicals used "y the contractor are to "e stored following !:3P1N; N13' policies ,see !ontact Person for details-. 8(. HA9AR,OUS MATERIALS 1. 3aterials containing designated su"stances as outlined in the :&H#1 will "e thoroughly evaluated "efore written approval is given to use and store these materials on site. *. 1ll products containing o5one$depleting su"stances ,:D#- are prohi"ited in the plant? manufacturing areas of !:3P1N; N13' at all times unless approved of "y the !ontact 'mployee and?or the '3# !oordinator. 1ctivities which re+uire the use of aerosol cans must "e discussed and approved "y the !ontact Person and a control plan regarding storage and usage implemented. .. 7hen using o5one depleting su"stances ,:D#- the !ontractor is responsi"le to send the !ontact 'mployee prior to the start of work a copy of their current license and verification from suppliers that :D# are in compliance. %he '3# Department will keep a copy of all :D# licenses on record for each pertinent contractor. /. 1ll tanks "rought on the !:3P1N; N13' premises containing fuel oil or any other ha5ardous products shall have secondary containment. %hese tanks must have proper la"eling and certification. :(. CLEANLINESS AN, ;ASTE MANA/EMENT 1. 1ll leftover material,s- will "e removed from !:3P1N; N13' property when the work?visit is completed and the area kept clean and free of de"ris. *. 1ll gar"age or waste material is to "e properly stored and disposed of at the end of each working shift unless arrangements have "een made otherwise. .. 1ll waste materials generated throughout the facility "y the contractor should "e discussed with the !:3P1N; N13' !ontact Person prior to the initiation of the pro=ect in order to make appropriate arrangements for proper disposal or recycling. /. !ontractors are re+uired to follow the !:3P1N; N13' recycling programs for the various waste streams they produce. 9nformation on availa"le recycling programs and arrangements to utili5e them must "e discussed with the !ontact Person. 0. 7aste must "e placed in the gar"age receptacles and in the case of demolition all scrap materials are to "e cleaned up on an ongoing "asis.

2. !ontractors will "e re+uired to remove profuse amounts of gar"age at their own e>pense unless otherwise stated "y the !ontact 'mployee. 4. 1ll waste sand concrete cement "locks etc. are to "e trucked offsite to an approved landfill site ,at contractors e>pense-. 1 copy of the site approval certificate must "e supplied the '3# !oordinator prior to removal. 6. !ontractors are re+uired to report any accidental spills "oth inside and outside of the facility immediately to the !ontact Person. %he !ontact Person will "e responsi"le for completing the 91P1 9nvestigation Report Form within */ hours. 8. !ontractors are responsi"le for the clean up and disposal of any spills at their own e>pense. 1(. 'mergency #pill kits are located in the following locations: Plant 1 location D shipping department Plant * location D shipping and receiving areas on the compressor me55anine Plant . location D #hipping ? Receiving area and "eside the #tamping #upervisors office. <(. E0UI-MENT 1. 1ny e+uipment "rought onto !:3P1N; N13' property must "e maintained and operated in accordance with appropriate regulations *. 1nyone operating moving e+uipment on !:3P1N; N13' property must show written evidence of appropriate training and?or certification. .. !:3P1N; N13'<s tools lift trucks hoists vehicles and cranes are to "e used "y competently trained contractors in specific situations and only with the e>press permission of the !ontact 'mployee. ).(. ENFORCEMENT 1. 'ach contractor employee will "e re+uired to sign and date this document stating that they have read and understood its contents and will comply. !ontractors are responsi"le for reviewing these regulations with each new employee "efore he ? she "egins work at the =o" site. %hese employees will not "e allowed to work at !:3P1N; N13' until this document has "een signed and dated. *. %he signed and dated portion of this manual will "e kept on file at !:3P1N; N13' and reviewed each time the contractor signs into the facility. 9f a signed and dated sheet for the !ontractor 3anual cannot "e found the contractor must read sign and date another copy. .. Ciolation of the rules contained in this document may result in the following actions: )st O==#ns#: +n& O==#nc#: 7ritten warning with re+uirement that the contractor detail safety?environmental action plan to prevent the offence form occurring again. their

7ritten notice of suspension for an appropriate period of time ,1 to / weeks- and action plan prior to recommencement of work. Notice of the second offence will "e sent to other !:3P1N; N13' !ompany facilities. 7ritten notice of suspension for an indefinite period of time. Notices will "e sent to all !:3P1N; N13' !ompany facilities.
