HERBAL TREATMENT PRESCRIPTION

Main formula

BAN XIA HOU PO TANG

Pinellia and Magnolia Bark Decoction. This formula is

especially useful when there is slight throat pain but there is an obvious feeling of a blockage or phlegm in the throat with plum-stone sensation. Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces, Jie Geng Radix Platycodi 6g, and Tu Niu Xi Radix Achyranthis 10g could be added into the prescription to increase the effect on relieving the throat pain.

DAO TAN TANG

Guide Out Phlegm Decoction. This formula has stronger


function than above one to eliminate Damp-Phlegm in the body and throat, but Pang Da Hai Semen

Sterculiae Scaphigerae 5 pieces, Jie Geng Radix Platycodi 6g, and Tu Niu Xi Radix Achyranthis 10g could be added into
the prescription to increase the effect to relieve the throat pain.

Modifications

1. If there is a sensation of fullness in the stomach, add Mu Xiang Radix Aucklandiae 10g and Bin Lang Semen Arecae 10g to promote the Qi circulation and relieve the fullness

If there is thick phlegm in the throat, add Yuan Zhi Radix Polygalae 10g and Qing Pi Pericarpium Citri Reticulatae Viride 10g to promote the Qi circulation and eliminate the phlegm 3. If the appetite is poor, add Cang Zhu Rhizoma Atractylodis 10g and Sha Ren Fructus Amomi 10g to dry Damp and improve the appetite 4. If there is a cough with phlegm, add Bai Jie Zi Semen Sinapis Albae 10g and Lai Fu Zi Semen Raphani 10g to resolve Phlegm and disperse the Lung-Qi 5. If there are loose stools, add Cang Zhu Rhizoma Atractylodis 10g and Bai Zhu Rhizoma Atractylodis Macrocephalae 10g to dry Damp in the Middle Burner.

ACUPUNCTURE TREATMENT Main points ST-40 Neiting, SP-6 Sanyinjiao, SP-9 Yinlingquan, LU-5 Chize, LU-7 Lieque, BL-2 Zanzhu, CV-12 Zhongwan, CV-23 Lianquan, LI-18 Futu and ST-11 Qishe. Reducing method is used on these points except on LI-18 and ST-11, to which even method is applied.

Explanations

This kind of throat pain is mainly due to formation of Damp-Phlegm in the body and throat, which has blocked the collateral in the throat causing the throat pain. Based on such pathology, treatment should focus on eliminating the Damp, resolving the Phlegm and relieving the blockage in the throat. In order to resolve Damp-Phlegm, some points should be applied to activate the Spleen and Stomach, since this kind of Damp-Phlegm is not caused by External invasion but by Internal disorder from the Spleen and Stomach. ST-40, the Connecting point on the Stomach channel, SP-6, the crossing point of the three Yin channels of the foot, and SP-9, the Sea point from the Spleen channel, are prescribed here together to activate strongly the Spleen and Stomach, eliminate the Damp and resolve the Phlegm. This is the root treatment. CV-12, the Gathering point for the Fu organs, helps the above points to harmonise the Fu organs and resolve the Damp-Phlegm in the body. Since the throat is dominated by the Lung, accumulation of Damp-Phlegm in the Lung and throat causes throat pain. Thus LU-5, the Sea point of the Lung channel, is used to eliminate Phlegm in the Lung and promote its physiological function. LU-7, the Connecting point of the Lung channel, is prescribed to disperse the Lung-Qi,

harmonise the collateral in the throat and relieve the throat pain. CV-23, LI-18 and ST-11, the local points, are used to benefit the throat and relieve the throat pain. In practice, it is very often seen that this kind of chronic throat pain can also be caused by accumulation of Phlegm in the nose, leading to downward flow of Damp-Phlegm behind the nose to the throat. In such cases, BL-2 is used to clear the nose and resolve the Phlegm in the nose.

Modifications

1. If there is a discharge of yellow phlegm from the nose and cough due to accumulation of PhlegmHeat in the Lung, add LU-10, the Spring point of the Lung channel, and LI-4 to clear Heat, eliminate Phlegm and relieve the cough 2. If there is a loss of the sense of smell and taste, add SI-18 and LI-20 and Extra Bitong, the local points, to clear the nose and promote its physiological function 3. If the throat is red and swollen, add LI-2, the Spring point of the Large Intestine channel, and LU-11, the Well point of the Lung channel, to clear the Heat and reduce the

swelling 4. If there is a feeling of fullness in the abdomen, a poor appetite and loose stools due to weakness of the Spleen and Stomach, add SP-3, the Source point of the Spleen channel, and SP-4, the Connecting point and the Confluence point of the Penetrating Vessel, to activate the Spleen and improve the digestion 5. Stagnation of Blood

Symptoms and signs The signs and symptoms of Blood stagnation are prolonged persistence of throat pain with a stabbing feeling, aggravation of the pain at night, swelling in the throat and a purplish colour, difficulty in speaking and swallowing, hoarseness, bleeding from the throat, or expectoration of phlegm with blood breaks, coughing and shortness of breath. Some patients might have had surgical operations for throat cancer. In some cases, there is a dark complexion and emaciation, a purple tongue, or purple

spots on the borders, and a choppy, deep, wiry or tight pulse. Principle of treatment Promote the circulation of Blood, eliminate the Blood stasis, harmonise the collateral and arrest the pain. HERBAL TREATMENT PRESCRIPTION

Main formulas

TAO HONG SI WU TANG and SHI XIAO SAN

Four-Substance Decoction with Safflower and Peach Pit and Sudden Smile Powder. The former formula can
promote the Qi and Blood circulation and eliminate Blood stasis, but it is not so effective at eliminating the Blood stasis, therefore, the latter formula is also added, which has the specific function of eliminating the Blood stasis. But still there are not enough herbs in these two formulas to promote the Qi circulation and to relieve the throat pain, so Chai Hu Radix Bupleuri 5g, Bai Shao Radix Paeoniae Alba 10g, Xiang Fu Rhizoma Cyperi 10g and Yu Jin Radix Curcumae 10g could be added. Meanwhile, Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces, Jie Geng Radix Platycodi 6g, and Tu Niu Xi Radix Achyranthis 10g could be added to increase the effect on throat pain.

XIAO YAO SAN and SHI XIAO SAN

Free and Relaxed Powder and Peach Pit and Sudden Smile Powder. The first formula can smooth the emotion,

regulate the Liver and promote the Liver-Qi circulation; the second formula can promote Blood circulation and eliminate the Blood stasis. Meanwhile, Xiang Fu Rhizoma Cyperi 10g, Pang Da Hai Semen

Sterculiae Scaphigerae 5 pieces, Jie Geng Radix Platycodi 6g, and Tu Niu Xi Radix Achyranthis 10g could be added to
relieve the throat pain.

Modifications

1. If there is profuse phlegm in the throat, add Chuan Bei Mu Bulbus Fritillariae Cirrhosae 10g and Gua Lou Pi Fructus Trichosanthis 10g to eliminate the Phlegm 2. If there is difficulty in swallowing, add Niu Bang Zi Fructus Arctii 10g and Wei Ling Xian Radix Clematidis 10g to regulate the Qi circulation and improve swallowing 3. If there is chest pain at night, add Yan Hu Suo Rhizoma Corydalis 10g and Dan Shen Radix Salviae Miltiorrhizae 10g to promote the Blood circulation, remove Blood stagnation and relieve pain 4. If there is a history of surgery, add Shui Zhi Hirudo 10g and Xian He Cao Herba Agrimoniae 10g to promote the Blood circulation and remove Blood stasis

5. In cancer patients, add Ban Zhi Lian Herba Scutellariae Barbatae 15g and Bai Hua She Shi Cao Herba Hedyotis Diffusae 20g to eliminate Toxin and relieve the pain

Patent remedy

Yan Hu Suo Zhi Tong Pian Corydalis Stop Pain Tablet. ACUPUNCTURE TREATMENT MAIN POINTS CV-21 Xuanji, CV-22 Tiantu, LU-7 Lieque, BL-17 Geshu, SP-6 Sanyinjiao, LI-4 Hegu, PC-6 Neiguan, LR-3 Taichong, LI-18 Futu and ST-11 Qishe. Explanations

Long-standing persistence of pathogenic factors in the body may cause stagnation of the Blood, leading to blockage of the collateral and slowing down of circulation in the channels in the throat, and causing a throat pain with a stabbing feeling and swelling BL-17, the Gathering point for the Blood, and SP-6, the crossing point of the three Yin channels of the foot, promote the Blood circulation and eliminate its stasis LI-4, the Source point of the Large Intestine channel, promotes circulation of the Qi and Blood in the upper body and relieves the pain LU-7, the Connecting point of the Lung channel, induces the treatment into the throat and disperses

the Lung-Qi so as to improve the voice PC-6, the Connecting point and Confluence point of the Yin Linking Vessel, regulates the Shen and calms throat pain 131 the pain. Modifications

1. If there is severe throat pain with aggravation at night, add ST-9 and ST-10 to circulate the Blood, eliminate the Blood stasis and stop the pain 2. In case of difficulty in swallowing and drinking, vomiting, add SP-4, the Confluence point of the Penetrating Vessel, and KI-27 to cause the Stomach-Qi to descend and relieve the blockage in the throat 3. If there is swelling in the throat due to stagnation of Blood, add ST-40, the Connecting point of the Stomach channel, to reduce the swelling and

eliminate blockage of the collateral in the throat 4. If there is restlessness due to severe throat pain, add HT-7, the Source point of the Heart channel, to calm the Heart and tranquillise the Mind 5. If there is expectoration of profuse phlegm, add SP-9, the Sea point of the Spleen channel, and CV-12, the Gathering point of the Fu organs, to activate the Spleen and Stomach and resolve Phlegm.

Deficiency of Yin of Lung, Heart and Kidney

Symptoms and signs Prolonged persistence of a slight throat pain, dryness in the throat, thirst especially at night, but a lack of much desire to drink, and sometimes a burning feeling in the throat. If due to deficiency of Kidney-Yin, the throat symptoms are often accompanied by night sweating, heat in the palms and soles of the feet, dry stools, scanty urine, and other symptoms such as tiredness, dizziness, tinnitus, poor memory, weakness in the knees and lower back pain. If due to deficiency of Lung-Yin, there is hoarseness, a dry cough, or cough with scanty Phlegm, or even blood breaks in the Phlegm. If due to deficiency of Heart-Yin, there may be insomnia, restlessness and palpitation. There is also a red tongue with a thin peeled coating and a rapid, thready and weak pulse. Main formula

SHA SHEN MAI DONG TANG

Glehnia and Ophiopogonis Decoction. This formula

is very good at nourishing the Yin of the Lung and Stomach, indicated in throat pain mainly due to deficiency of Yin of the Lung and Stomach. Sheng Di Huang Radix Rehmanniae 10g and Xuan Shen Radix Scrophulariae 10g could be added into the prescription to promote the secretion of the Body Fluid and nourish the Yin of the Lung and Stomach. Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces and Jie Geng Radix Platycodi 6g could be added into the prescription to increase the effect on throat pain.

Alternative formulas

QING ZAO JIU FEI TANG

Eliminate Dryness and Rescue the Lung Decoction. This

formula has the same functions as the one above, but its tonifying effect is stronger. The same modifications as above should be applied. QI JU DI HUANG WAN Lycium Fruit, Chrysanthemum and Rehmannia Pill. This formula

is indicated in deficiency of Yin of the Kidney and same modifications as the above one should be applied. ZHI BAI DI HUANG WAN

Anemarrhena, Phellodendron, and Rehmanniae Pill.

This formula is slightly stronger than QI JU DI HUANG WAN at nourishing the Yin and clearing the Deficient Fire, and the same modifications as above should be applied.

Modifications

1. If there is night sweating, add Wu Wei Zi Fructus Schisandrae 10g and Mu Li Concha Ostreae 10g to arrest the sweating 2. If there is insomnia, add Suan Zao Ren Semen Ziziphi Spinosae 10g and Bai Zi Ren Semen Biotae 10g to nourish the Heart-Yin and calm the Shen 3. If there are dry stools, add Shou Wu Radix Polygoni Multiflori 10g to nourish the Yin, promote the defecation and relieve the dry stools 132 head and neck pain 4. If there is heat in the palms and the soles of the feet, add Hu Huang Lian Rhizoma Picrorhizae 10g and Qing Hao Herba Artemisiae Chinghao 10g to clear the Deficient-Fire 5. If there is lower back pain and weakness in the knees, add Sang Ji Sheng Ramulus Loranthi 10g to strengthen the Kidney and relieve the lower back pain 6. If there is a dry mouth, add Shi Hu Caulis Dendrobii to nourish the Yin and relieve the thirst.

Rehmannia Pill.

Explanations

The Kidney rules water and the Yin in the body. Chronic throat pain due to deficiency of Yin is mostly related to the Kidney. Thus nourishing the Kidney-Yin should be the root treatment for such cases. KI-7 and LU-8, the Metal points of the Kidney channel and Lung channel respectively, have the function of nourishing the Yin and promoting the secretion of Body Fluids so as to benefit and moisten the throat SP-6, the crossing point of the three Yin channels of the foot, helps the above two points tonify the Yin and relieve dryness in the throat. This is the root treatment. LU-5, the Water point of the Lung channel, directly promotes the secretion of Body Fluids in the Lung system and moistens the throat. Application of LU-7 and KI-16 simultaneously is a special combination to open the Directing Vessel,

moisten the throat and sedate the throat pain. HT-3, the Water point of the Heart channel, nourishes the Yin and calms the Shen. CV-23, LI-18 and ST-11, the local points, are used to benefit the throat and relieve the throat pain.

LU-1 Zhongfu, LU-6 Kongzui, LU-11 Shaoshang, ST-9 Renying, ST-10 Shuitu, ST-42 Chongyang, HT-6 Yinxi, LR-8 Ququan and KI-10 Yingu, etc.

Modifications

1. If there is dizziness, poor memory, lower back pain, and weakness of knees, add KI-3 and KI-10, the Source point and the Sea point of the Kidney channel respectively, to tonify the Kidney-Essence and strengthen the lower back 2. If there is night sweating, heat in the palms and soles of the feet and thirst due to deficiency of Yin, add HT-6, the Accumulation point of the Heart channel, and KI-2, the Spring point of the Kidney channel, to nourish the Yin and clear the DeficientHeat 3. If there is a dry cough, hoarseness or blood breaks in the phlegm, add BL-13, the Back Transporting point of the Lung, and LU-6, the Accumulation point of the Lung channel, to nourish the Lung-Yin and relieve the cough and bleeding 4. If there is insomnia, restlessness and palpitations, add BL-15, the Back Transporting point of the Heart, and CV-14, the Front Collecting point of the

Heart, to nourish the Heart-Yin and calm the Shen 5. If there is nervousness, headache and irritability due to deficiency of Liver-Yin with hyperactivity of Deficient-Fire, add LR-3, the Source point of the Liver channel, and GB-20 to calm the Liver and reduce the Deficient-Fire.

Deficiency of Qi of Lung, Spleen and Kidney Symptoms and signs

A chronic history, slight throat pain, a feeling of cold in the throat, hoarseness, a low and weak voice, aggravation of the pain with tiredness, no swelling in the throat, sometimes a slight white phlegm in the throat are the symptoms and signs of Qi deficiency in the Lung, Spleen and Kidney. Also a pale and swollen tongue with tooth marks and a wet and watery coating, and a thready, deep and slow pulse. If this kind of throat complaint is accompanied by a pale complexion, spontaneous sweating, aversion to cold, cold hands and feet and other symptoms, such as listlessness when speaking, tiredness, a tendency to catch colds easily and a slight cough with expectoration of white phlegm, it results from deficiency of Lung-Qi. If there is a poor appetite, loose stools, abdominal distension, nausea and weakness of the muscles, it results from deficiency of Spleen-Qi. If there is general tiredness, lower back pain, weakness in the knees, nocturia, impotence and profuse and clean urine, it results from

deficiency of Kidney-Qi.

Principle of treatment

Main formula

BU FEI TANG

Tonify the Lung Decoction. This formula is indicated in deficiency of Lung-Qi. Xing Ren Semen Pruni Armeniacae 10g and Zi Su Zi Fructus Perillae Frutescentis

10g could be used together with this prescription to eliminate Phlegm and stop the cough. Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces and Jie Geng increase its effect on throat pain.

Radix Platycodi 6g could also be added to the prescription to Alternative formulas


BU ZHONG YI QI TANG

Tonifying the Middle and Benefiting Qi Decoction. This

formula is very good at tonifying the Qi in the Middle Burner but, as it is less efficient at relieving the throat pain, some other herbs should be added to harmonise the collaterals, such as Pang Da Hai Semen Sterculiae Scaphigerae 5 pieces, and Jie Geng Radix Platycodi 6g could also be added into the prescription to improve

its effect on throat pain. Meanwhile, Gui Zhi Ramulus Cinnamomi Cassiae 10g could also be used to warm the Interior and dispel the Cold.
