BRIEF HISTORY
$n recognition of the vast potential of the "hilippines in terms of manpower resource, USST was established in September of 0++( as a technical and vocational training school! The founders sought to provide students the opportunity to have formal but low.cost education, and ultimately produce graduates who become valuable members of society, to the benefit of not only the student and his@her family, but also the community and the country! Starting out small, with only ten &0'* teachers and four hundred &(''* students when it was established in SA 0++(.0++), enrollment has steadily increased yearly! $n 0++/, under new management, the school underwent significant changes! Under new leadership, the school revised its curricula offeringsB e panding its original course offerings to ( year degree programs while still retaining its original technical and vocational courses! The school has even branched into such fields as criminology, hotel and restaurant management, and social wor%, offering bachelor degree programs, technical @ vocational certification programs, and technical @ vocational ladderized programs! Today, the USST 3oard of Trustees headed by ?r! 6uillermina C! Tabamo continues to reaffirm its commitment to play a bigger role in developing and cultivating the country#s human resources and to %eep true to its missionD to produce competent graduates and productive citizens in support of the nation#s economic and social development agenda!

Late *e'(&t*at(on or registration after the official period for registration and enrollment as specified in the academic calendar may be allowed only upon reasonable and 8ustifiable circumstances to be determined for approval by the 5ice "resident for 9cademic 9ffairs! 9 late registration fee will be charged as a penalty! $n no case shall late registration be allowed after si percent &1I* of the regular class meetings have been held! 1! 9ny student who wishes to .*o&&-*e'(&te* or .*o&&-en*o%% in another accredited institution of higher learning may be authorized to do so upon recommendation of the ?ean@?epartment 2ead concerned and the College :egistrar with the approval of the 5ice "resident for 9cademic 9ffairsB provided, that the sub8ect&s* he@she needs to enroll in during a given term is@are not offered in the College! /! Fo .*o&&-en*o%%ee from his institution shall be admitted into the College without the written permission from his@her 4other School#s :egistrar#s 7ffice! The permit shall state the total number of units for which the student is registered and the sub8ect&s* that he is authorized to ta%e in the College!

ACADE!IC AFFAIRS 1$ STUDENT CLASSIFICATION 0! A..o*d(n' to a.ade+(. %oad and &tatu&/&tand(n': students are classified as followsD Regular Student J one who is registered for formal academic credits and carries the full load required in a given semester by his@her curriculum Irregular Student J one who is registered for formal credits but carries less than the full load required in a given semester of his@her curriculum, or load requirements not prescribed under the curriculum!

2$ ACADE!IC LOAD 0! The total number of units for which a student may register shall be in accordance with the curricular program he@she is enrolled in! 7ne academic unit is the equivalent of at least eighteen &0,* lecture hours or the corresponding laboratory hours! The amount and content of the study is prescribed in the curriculum! 9ny change therefrom shall be made only upon approval of the :egistrar! -! ?uring the summer term, the normal load carries twelve &0-* units! $f graduating in the summer, the 5ice "resident for 9cademic 9ffairs may allow a student to ta%e fifteen &0)* units! E! Fo student shall be allowed to ta%e more than the number of units specified in his@her curriculum e cept for graduating or academically outstanding students! $n such cases, only an e cess of si &1* units shall be allowed! (! 9ll requests for overload shall be sub8ect to critical evaluation of the ?epartment 2ead@?ean, recommendation of the College :egistrar and final approval of the 5ice "resident for 9cademic 9ffairs!

A..o*d(n' to ;ea* %e<e%, students are classified as followsD Freshman J one who is in the 0st year of his@her curriculum, or who has not finished the prescribed sub8ects of the first year of his@her curriculum, or twenty.five percent &-)I* of the total number of units required in his@her entire course! Sophomore J one who is in the -nd year of his@her curriculum, or who has satisfactorily completed the prescribed sub8ect of the first year of his@her curriculum or has finished more than twenty. five percent &-)I* but less than fifty percent &)'I* of the total number of units required in his@her entire course! Junior J one who is in the E rd year of his@her curriculum, or who has completed the prescribed sub8ects of the first and second years of his@her curriculum or has finished more than fifty percent &)'I* but less than seventy.five percent &/)I* of the total number of units required in his@her entire course! Senior J one who is in the (th year of his@her curriculum, or who has completed the prescribed sub8ects of the first, second and third years of his@her curriculum or has finished seventy.five percent &/)I* or more of the total number of units required in his@her entire course!

)! The ?epartment 2ead@?ean may limit the academic load of full.time


or part.time wor%ing students employed outside the College, upon the submission of a certificate of employment to be issued by the student#s employer! 3$ CHAN"IN" OF SUB#ECTS: SUBSTITUTION: DRO00IN" USST recognizes the need to change sub8ects in order to facilitate immediate and successful completion of their course curriculum! >i%ewise, the need for dropping of sub8ects and@or withdrawal of enrollment is also allowed, provided, it is done in accordance with the procedural guidelines on the matter! Ru%e& and Re'u%at(on&3 On C2an'(n' and/o* Sub&t(tut(on o- Sub=e.t&3 0! 9dding or changing of sub8ects shall be allowed for valid reasons but only within the period of two &-* wee%s after the opening of classesB -! The prerequisites of the sub8ects added or changed have already been ta%en with a passing gradeB

E! The total number of units after the addition or change shall not e ceed the ma imum units prescribed in the course curriculum of the student per semester or year levelB (! Sub8ects changed@added unofficially or without prior approval of the ?ean shall not be given credit! )! Substitution of sub8ects may be authorized in any one of the following casesD a! 9 curriculum has been superseded by a new one and the substitution tends to bring the old curriculum in line with the new one! 2owever, in case of curricular revision, the student follows the curriculum he was enrolled in while in the first year! b! There is a conflict of schedule between required sub8ects! c! The required sub8ect is not offered! 1! "etition for substitution should beD a! =or sub8ects within the same department if possibleB if not, the proposed substitute must be allied to the one being substituted! b! 3etween sub8ects of the same number of units! c! :ecommended by the class adviser and approved by the 5ice "resident for 9dministration! d! Submitted to the 5ice "resident for 9dministration during the registration period or before the opening of classes! e! 9cted upon by the College ?ean in case the petition is disapproved, the student may appeal to the < ecutive 5ice "resident, whose decision on the matter is final! /! 9ll change and@or addition of sub8ects shall be done through written request of the student indicating therein the reason for the request duly noted by the guardian or parent and concerned instructor@s! On D*o55(n' and/o* 4(t2d*a1a% 0! ?ropping of sub8ects for the purpose of transferring to the class of another teacher handling the same sub8ect shall not be allowed, e cept in cases where there is proven conflict in the schedule of the student! =or this purpose, only a written consent of the faculty members concerned, who shall facilitate the student transfer in the master#s list, is necessary!

-! ?ropping period J ?ropping a sub8ect shall be allowed within 0 4onth after the start of the regular classB however, a student may be allowed to drop a sub8ect only for valid and 8ustifiable reasons! 7therwise, withdrawal of enrollment after 0 month shall mean a failing grade for a student! E! 9 student who wishes to add or drop a sub8ect should secure the 9dd@?rop =orm from the :egistrar#s 7ffice which should be properly accomplished (! =or enrollment withdrawal, the Withdrawal =orm shall be filled out and signed by the student noted by his@her parent or guardian! )! Sub8ects dropped after the preliminary e amination will be mar%ed K?:"H &dropped* in the Students# Transcript of :ecordsB 1! 9ny and all forms of dropping and adding of sub8ects at the instance of any student shall be charged with a corresponding fee! >$ SHIFTIN" OF COURSES Ru%e& and Re'u%at(on& 0! The classification of a student shifting to another course or program at any given semester shall be determined by the number of units or sub8ects completed and passed that can be credited to the new program! -! Students who stopped school unofficially may be allowed to shift to another program@course upon their re.enrollment provided he@she meets all the academic requirements of the new course! E! Sub8ects ta%en at USST or those ta%en from other schools may be credited provided that the sub8ects can be equated to the curricular requirements of USST! <valuation shall be done by the ?eans or the ?epartment 2eads of the colleges concerned, verification by the :egistrar and approval by the 5ice "resident for 9cademic 9ffairs! (! $n general, no substitution shall be allowed for ma8or and mandated sub8ects prescribed in the curriculum in which the student has failed, e cept when, in the opinion of the ?ean or ?epartment 2ead of the College, the proposed substitute is substantially similar in sub8ect matter to the required sub8ect or a new one has superseded the e isting curriculum! $n case of curricular revision, however, the student shall follow the curriculum he@she used during his@her initial and first year of enrollment with USST!

)! 9 student may transfer from one curriculum to another curriculum of the college provided thatD The ?ean or ?epartment 2ead approves his@her release from the College in which he@she is enrolled! 2e@she satisfies the requirements of the College in which he@she intends to enroll! There is an available slot for an additional student in the college he@she wishes to transfer to! 0*o.edu*e 0! =ill out the appropriate form -! Submit to the College :egistrar for verification E! <ndorse to the ?ean or ?epartment 2ead concerned for approval (! "roceed to the 9ccounting office for payment of corresponding processing fee! ?$ CLASS ATTENDANCE :egular and punctual attendance is required of all students from the first meeting of every course as it is essential for satisfactory academic performance! Ru%e& and Re'u%at(on& 0! 9ny student who, for any unavoidable cause, is obliged to absent himself@herself from any College e ercise must obtain a certification or an e cuse slip from his@her attending physician or parent, to be presented to his@her professors! -! <very e cuse slip must be presented to the professor@s within two &-* days, to be counted from the day a student returns to his@her class! =ailure to comply with this provision shall cause any absence to be une cused! E! < cuses are for time missed only! 9ll wor% covered by the class during the student#s absence shall be made up by the student to satisfy the requirements of the course, within a reasonable period of time, as required by the teacher concerned! (! 9ny student who, during the semester, has for any cause been absent for more than twenty percent &-'I* of the required number of class and laboratory periods or any other scheduled wor% in a given semester shall be dropped from the course! )! $f the ma8ority of the absences are not e cused, the student shall be given a grade of =?9 &=ailure ?ue to 9bsences*B but if the ma8ority of the absences are e cused, the accumulated absences shall be ta%en into account as deduction in his@her grades, which deductions may range from light, medium, or heavy, depending upon the number of absences, as well as the attendant circumstances! 1! < cused absences such as attendance to departmental meetings, conferences, athletic activities and the li%e where the student is sent as school representative are not counted against the allowable number of absences! /! Time lost due to late enrollment shall be considered as time lost by absence! ,! 9ny student, who, without permission of the professor, leaves the classroom for the remainder of the class period or stays out for more than fifteen &0)* minutes, will be mar%ed absent! +! 9 student shall be considered absent from class if he is not in the designated classroom within the first third fraction of the scheduled class time or within the firstD . 1' minutes of a E hour class period . E' minutes of a 0!) hour class period . -' minutes of a 0 hour class period 0'! 9 student is considered late or tardy if he@she arrives in class within the time schedule described in the previous paragraph &Fo! +*! Three incidences of tardiness shall be considered as one &0* absence! 00! $n cases where the professor is late, students should not leave the classroom until the first third fraction of the scheduled class time has passed! Students who leave earlier than this prescribed period may be mar%ed absent by the professor! $f the professor does not come within the prescribed period, students are e cused from class instruction!

0-! $n case of absence from a final e amination, the student concerned may arrange for a ma%e.up e am! Those whose class standing is failing, forfeit credit for the sub8ect and shall be given a final grade of )!'! B$ EAA!INATIONS ;uizzes and e aminations, given periodically to chec% the learning progress of students, shall be prepared and administered by the instructor concerned! 2owever, in case the instructor is absent, a proctor may be allowed to administer the e amination sub8ect to the approval of the ?ean or the ?epartment 2ead! Ru%e& and Re'u%at(on& 0! There are four &(* periodic e aminations each regular semester, namelyD the "reliminary, 4idterm, "re.=inal and =inal e am! -! 9 59>$? <G94$F9T$7F "<:4$T issued by the 9ccounting 7ffice is required for ta%ing the periodic e ams! E! 9ny student who fails to ta%e any scheduled e amination due to 8ustifiable causes &e!g! serious illness, death of family members* shall be given a special e amination but only upon approval of the ?ean or ?epartment 2ead and payment of a special e amination fee per sub8ect at the 9ccounting office and presentation of the official receipt to the instructor concerned! The student shall be given seven &/* school days to complete the e aminations otherwise a failing grade shall be given for that particular e amination! (! $ssuance of all e amination permits shall be done by the 9ccounting 7ffice upon payment of the scheduled amount of the installment fees, as followsD 0AY!ENT SCHEDULE <ntrance =ee &fi ed* 0st $nstallment -nd $nstallment Erd $nstallment DEADLINE Upon registration 3efore "reliminary < am 3efore 4idterm < am 3efore "re.=inal < am

)! $n case of =U>> "9A4<FT $F C9S2, a 1?C d(&.ount on basic tuition fee will be granted and all four &(* e amination permits shall be issued to the student one &0* wee% before the scheduled e amination! 1! $n case of loss of e amination permit, the student shall go to the 9ccounting 7ffice for the issuance of a duplicate copy after having accomplished the appropriate form for such and upon payment of the corresponding replacement fee! /! The 9ccounting 7ffice, sub8ect to approval by the 5ice "resident for 9dministration or the < ecutive 5ice "resident as the case may be, may issue a provisional < amination permit under 8ustifiable circumstances! D$ SETTLE!ENT OF OUTSTANDIN" OBLI"ATIONS While the school recognizes the financial conditions of students who are unable to readily pay their tuition and other fees, students and their parents are e pected to settle their obligations in full as soon as possible! Students who fail to pay on the scheduled payment dates shall incur penalty for late payment, as well as interest charges, if applicable! -! =or special cases, the parent or legal guardian of a student may be allowed to e ecute a promissory note to pay the outstanding balance of tuition@fees on a date other than the period provided in paragraph ( under < aminations! =ailure to pay on the date indicated in the promissory note shall incur an additional fine e clusive of interest, and may merit other sanctions, if warranted! E! >i%ewise, graduating students who still have unpaid accounts shall settle their outstanding obligations 3<=7:< the deliberations on candidates for graduation! 7therwise, they will not be allowed to participate in the commencement e ercises and their credentials &?iploma, Transcript of :ecords, etc!* shall be withheld from them until settlement of their outstanding obligations!

The school recognizes the academic freedom of teachers to design and implement their respective grading system! 2owever, a student may appeal the computation of his or her grade to the teacher concerned, then the ?epartment 2ead or ?ean, and finally the 5ice "resident for 9cademic 9ffairs! -! =or ?egree Courses, Class Standing may be composed of :ecitation, ;uizzes, 9ttendance, ?ecorum@9ttitude and "ro8ects@9ssignments! The distribution of the percentage for each criteria shall be ad8usted to the requirements of the particular department as recommended by the ?ean concerned! This is to encourage the e ercise of freedom in academic instruction! E! =or Trade, Technical@5ocational "rograms, Class Standing shall be composed of S%ills, :ecitation, ;uizzes, 9ttendance, ?ecorum@9ttitude and "ro8ects@9ssignments! 3$ 2$ STUDENT OR"ANIGATIONS -!0 9 student organization is a group of students which the membership maybe voluntary and spontaneous or involuntary and automatic! 5oluntary and spontaneous organizations include fraternities, sororities, special interest groups and similar groups bound by common ob8ectives and interests! ?epartmental organizations are those whose membership is made up of students belonging to a department, and whose faculty adviser is a faculty member of that department!

RECO"NITION OF STUDENT OR"ANIGATIONS E!0 9ny group of ten &0'* students desirous to form a student organization may apply for recognition of such organization with the 5ice "resident for Student 9ffairs! The application for recognition shall be sub8ected to the following proceduresD E!0!0 9 letter of application is to be filed at the 7ffice of the 5ice "resident for Student 9ffairs accompanied by the followingD

$n addition to the rules and regulations under this part, the establishment and operation of student organizations at the College are governed by the rules and regulations established by the Commissioned on 2igher <ducation &C2<?*! Students desiring to establish, 8oin, participate in student organizations on campus shall do so as a right, sub8ect only to reasonable regulations promulgated by the College through the 5ice "resident for Student 9ffairs! $t is also understood that

e! Fame of the faculty adviser with his@her letter of acceptance addressed to the 5ice "resident for Student 9ffairs! E!0!9t no time in the organization#s e istence should there be less than ten &0'* active members in the said organization! The membership of the said organization shall 7F>A be bona fide students of the College!

E!+ Rene(al of Recognition) Student organizations which have been previously granted recognition are required to fileD a! 9 formal letter of intent to see% renewal of recognition and to submit other documents which may be required by the 5ice "resident for Student 9ffairs! b! 9 new list of officers with their names, respective, positions, courses, curriculum year, section and specimen signaturesB and c! Fame of faculty adviser@s with a letter of acceptance addressed to the 5ice "resident for Student 9ffairs!

E!- Constitution and %y&'a(s! <ach student organization shall have a Constitution and 3y.laws which shall be approved by the 5ice "resident for Student 9ffairs! Fo such organization shall be allowed to function without prior approval! E!E The period of application for recognition or accreditation of all student organizations shall be announced by the 5ice "resident for Student 9ffairs within E wee%s after the start of the school year! E!( The recognition of student organizations shall be a precognition for its operation in the College! E!) 9 certificate of recognition shall be issued by the 5ice "resident for Student 9ffairs to each student organization upon full compliance of the requirements above prescribedB however, such certificate shall be effective for one school year only and will have to be renewed each succeeding year that the organization is in e istence! E!1 9 newly organized student organization shall be placed on a three.month period of probationary status prior to recognition! ?uring such period, it shall be issued a permit to operate! E!/ 9ny student organization, which does not renew its recognition for a period of one school year, shall be considered inactive and maybe given a chance to renew its recognition provided that it submits a written e planation of its failure to reapply! 9ny student organization that fails to renew its recognition for more than one school year shall be considered defunct and has to undergo the same procedure as specified under Section (!0 of this chapter! E!, 9ny student organization, which has been granted recognition and violates its own statement of purpose@constitution, is liable to revocation of its certificate of recognition after due process!

E!0' <ach student organization shall have a faculty adviser whose acceptance of the position of adviser must be done in writing and filed with the 5ice "resident for Student 9ffairs!

E!00 The faculty adviser has the following duties and responsibilitiesD a! 9ttends meetings of the organization upon request of the sameB

b! $s available for consultation to all members of the organization, especially to the officersB c! $f the adviser is unable to attend the meeting, s@he should ensure that s@he is informed about the matters ta%en up during the meetingB

d! "rovides guidance in the planning of activitiesB e! 9ttend and monitor the organization#s activity if held off. campus or when the organization is representing the schoolD 0! $f the adviser is unable to attend, he should request another faculty member to ta%e his@her placeB -! The adviser in.charge should ensure the safety of all members of the organization and all others who may be involved in the activity!

f! >iaises with the 5ice "resident for Student 9ffairsB E!0$t is presumed that the faculty adviser is aware of the activities of the student organization and shall be responsible for the organization and the actions of its members, which should always be in accordance with these rules! >$ STUDENT ACTI ITIES 0! Student organizations may organize and hold social functions, cultural, educational, spiritual@religious or sports activities! =or activities to be held inside the campus, the proponents must observe the following procedureB a! b! Secure and accomplish the application form to hold an activity and request for use of a campus facilityB The signatures of the organization#s representatives and the faculty adviser are pre.requisites! $f any of the two &-* signatures are absent, the 5ice "resident for Student 9ffairs will not entertain or allow the application for the pro8ectsB Submit the accomplished form for final approval to the 5ice "resident for Student 9ffairs!

$n the interest of maintaining the academic atmosphere in the campus, activities outside the time slot provided for in the preceding section shall be conducted in enclosed areas, so as not to disturb normal classroom sessions!

)! 9ctivities should be scheduled at such time and place that would not disrupt normal classroom sessions! 1! :ooms should be reserved at least five &)* days from the date of the intended activity so that the proper permit can be issued and the proper preparations for the venue can be ensured! :equests for equipment such as the use of the sound system, podium, etc! by student organizations and outside groups are also channeled through the 5ice "resident for 9dministration! The requests should be received at least one &0* wee% before the intended activity! /! The College will not grant permission for the use of campus facilities when such use will result in clear and present danger and damages of propertyB commission of crimeB occasions which are in substantial opposition to public policy, customs and traditions! ,! College recognized activities or gatherings sponsored by students and conducted after )D'' p!m! and those scheduled during non.class days must be attended by a faculty adviser!

Secure the form from the 5ice "resident for 9dministration and fill in the necessary information and signaturesB and, b! Submit the completed form in triplicate for the approval of the head of 5ice "resident for Student 9ffairs who will retain the duplicate copy upon approval! E! U&e o- Ca+5u& Fa.(%(t(e&! Student representatives of recognized student organizations may request authorization from the 5ice "resident for Student 9ffairs to sponsor academic, cultural, athletic, spiritual or social activities! These groups may avail themselves of campus facilities for their club or class activities but only after securing the endorsement of the 5ice "resident for Student 9ffairs and approval of the concerned offices! Class section activities must be approved by their respective ?epartment 2eads or ?eans!

+! Fo group activity or gathering beyond 1D'' p!m! shall be allowed inside the campus e cept upon clearance from the 5ice "resident for 9dministration! 0'! Two &-* wee%s before the start of the final e aminations, a ban on all activities shall be enforced! This is to ensure that all students are given enough time to review for their e amination! 00! "osting and announcements! 9ll postings of student organizations inside the campus should bear the approval or approval stamp of the 5ice "resident for 9dministration! "osting includes notices of organizational meetings, assemblies, announcements, messages and the li%e, provided that they are not contrary to law and school policy! 0-! The 5ice "resident for 9dministration and the Security ?epartment have the right to remove posters@announcements without prior approval! 0E! The use of cloth streamers and the li%e should also bear the stamp of approval! The standard duration of this type of posting is eight &,* days only!

The 5ice "resident for 9dministration has the right to disapprove requests for posting on the bulletin boards if the requesting organization has already violated the above rules@ regulations three &E* times! 0)! The 5ice "resident for 9dministration has the right to KremoveH posted materials to give way to emergency or important announcement from the College or from the government! ?$ "UIDANCE AND COUNSELIN" OFFICE $t provides a broad spectrum of counseling and psychological services designed to assist students in channeling their creative energies and in achieving more effective personal, social, educational, and vocational development! $t offers the following servicesD E!0 "sychological testing and personality evaluation, E!- 9cademic advising and career counselingB and E!E 6roup counseling, >ife planning and peer counseling B$ "UIDELINES ON THE 0RESENTATION OF CULTURAL 0RO"RA!S AND ACTI ITIES 0! -! <very proposed cultural program or activity shall be in pursuit of the ob8ectives of the 9cademe! <very cultural program or activity to be held in the College shall have prior clearance from the 7ffice of the "resident through the 5ice "resident for 9dministration! 9ny bona fide faculty, administrative employee, or recognized campus. based student organization that desires to present a cultural program or activity shall submit the following to the 5ice "resident for Student 9ffairsD letter of request, tentative program, production staff, budget estimates, source of funding, and copy of the program! The letter of request shall state the title and brief description of the activity, ob8ective&s*, inclusive date and time, venue of the presentation, and other pertinent information! $t must also bear the signature&s* of the authorized representative&s* and endorsing@approving head&s* of offices or advisers! $n case of departmental organizations, the ?epartment 2ead@?ean shall act upon the letter of request! )!0 $t shall ma%e the necessary recommendation to the 5ice "resident for Student 9ffairs! )!- $t shall inform the requesting organization if the activity is approved or not!

The pro8ect proponents shall inform the 5ice "resident for Student 9ffairs of intended changes in the pro8ect within two days prior to its presentation! 7ne wee% after the activity they shall submit the following for documentation or record purposesD 1!0 1!1!E 1!( Three copies of the actual programB 9t least one picture of the presentationB "ress release, newspaper reports about the activity, if anyB =inancial report &in case of a fund raising pro8ect*!

$n the case of fund.raising activity, the pro8ect proponents shall remit to the 5ice "resident for =inance at least 0'I of the net proceeds!

7ther organizations and agencies outside the College may use the facilities of the College for their activities by arrangement with the 7ffice of the 5ice "resident for 9dministration sub8ect to e isting policies, rules and regulations of the College!
