Thank you for considering Peso Affluence Gold, a single-pay variable life insurance product that gives you both living and death benefits. Your living benefit is the account value of your policy. The death benefit on the life insured is the higher of the Account Value or the policy's Face Amount adjusted by all partial withdrawals and top-ups made, subject to minimum death benefit.* As your policy is a variable contract, the Account Value may increase or decrease in accordance with the investment experience of the separate investment funds** that your policy is linked to. AS THE POLICYOWNER, YOU SHALL SOLELY BEAR ALL THE INVESTMENT RISKS UNDER THIS POLICY. Yields depend on interest and foreign exchange rate levels, both of which may fluctuate. Other factors that affect yield include changes in the credit standing of the issuers and changes in the value of the stocks and dividends received. Thus, the performance of the separate account(s) is not guaranteed and the value of the policy could be less than the capital invested subject to any specified minimum guarantees. Past performance of the Funds is not necessarily indicative of future performance.

The Low, Medium and High values are calculated assuming fund earning rates of 4.00%, 8.00% and 10.00%, respectively, net of Fund Management Fees. These rates are used merely to illustrate and do not represent actual year-on-year performance. These scenarios DO NOT represent lower and upper limit of fund crediting levels. The values shown are not guaranteed and depend on the actual performance of the fund. The Account Value above are net of: a) Premium Load of 4% is deducted from the Basic Premium received. b) Bid-Offer Spread of 1.5% is the spread between Bid Price and Offer Price. The Bid Price is 98.5% of Offer Price. c) Management Fee of 2.00% per annum but not to exceed 3% covering the cost of managing the assets. d) Policy Fee* initially equal to PHP 6.67 per month for this Fund. e) Cost of Insurance* starts at an annual rate of 2.38 per thousand of amount at risk or PHP 1.21 per month and increases with age but will not exceed a monthly charge of 83.33 per thousand of amount at risk for standard risk class. Cost of Insurance varies by age, gender and risk class. *These will be deducted monthly from the Account Value at Bid Price. The deduction will be charged to each Fund in proportion to the Account Value of each Fund. This illustration is based on annual mode and standard risk class. The figures in the illustration above are based on the assumptions in the accompanying first page (Main Page) of this illustration. The company reserves the right to change the charges upon approval of the Insurance Commission and by giving the Owner at least three (3) months notice.

The Investment Funds ("Funds") mentioned in this document are specific to variable life insurance contracts and are not considered mutual funds. The Variable Life policy holder shall bear all investment risks. Past performance of the Funds is not necessarily indicative of future performance. Yields are not guaranteed. Yields indicated are after final tax and are exclusive of charges associated with the variable life policy. The Unit Price of a particular Fund is computed by dividing the Net Asset Value of the Fund by the number of outstanding units of the Fund on the Valuation Date. Unit Prices of the Funds are published weekly in major newspapers. Certified true and Correct:

VARIABLE LIFE INSURANCE PLAN ACKNOWLEDGEMENT OF VARIABILITY

Applicant : IVAN ANGELO S. APOSTOL Application No : Financial Adviser's Name : NORMA D. MENDIOLA

Code No : 329259

Unit :

I acknowledge that: I have applied with The Manufacturers Life Insurance Co. (Phils.), Inc. for a Variable Life Insurance Policy, and have reviewed the illustration(s) that shows how a variable life insurance policy performs using company's assumptions and based on Insurance Commission's guidelines on interest rates. I understand that since the fund performance may vary, the values of my units are not guaranteed and will depend on the actual performance at that given period and that the value of my policy could be less than the capital invested. The unit values of my Variable Life Insurance are periodically published. I understand that the investment risks under the policy are to be borne solely by me, as the policyholder.

UNDERSTANDING OF GUARANTEED INSURABILITY OFFER

I understand that: Guaranteed Insurability Offer (GIO) is a program designed to provide insurance protection, with no medical examination required, subject to certain issue limits and underwriting guidelines. The product is offered under Guaranteed Insurability Offer (GIO) subject to limits set by the Company. Should my total insurance coverage under GIO with the Company exceed such limit, the Company will decline my application under the GIO. In such event, I can apply for the insurance coverage exceeding the GIO limits using the Application for Variable Life Insurance and undergo the Company's regular underwriting process. GIO does not mean guaranteed approval of my application. My application may be declined for underwriting reasons such as but not limited to the following: 1) I have exceeded the GIO limits set by the Company, 2) I have not submitted the complete Anti-Money Laundering Act (AMLA) requirements, 3) I have not provided complete information in my GIO application form, 4) The health declaration is not acceptable according to the Companys underwriting guidelines, or 5) I have previous application with Manulife Philippines or other life insurance companies which was deferred, postponed OR declined (regular underwriting or simplified issue), regardless of reason.
