Case Conceptualization Outline COUN 7737 Practicum Spring 2010

Client: Jeremy Diggers__________________

1. Identifying Data and Background Information: Caucasian male in his late twenties or early thirties. Has a fianc, an ex-wife, and five children. He is close with the two children who he had with his fianc. The daughter that he had with his exwife does not know that Jeremy is her father. The remaining two children have not come into conversation yet. He is a recovering alcoholic, having used alcohol as his way of coping with negative emotions. This is our first session and we plan to meet weekly. 2. Presenting Problem: The client comes into counseling to talk. Presenting problems include dealing with his recovery process. However, I have noticed that he has very strong reactions to the concepts of self-love, mixed feelings towards his stepfather, and dealing with his emotions. I believe that these are underlying concerns that he does not recognize yet. I plan to ease him into discussing these topics starting with his father and later going into self-love. This will take a few sessions, but I believe that due to his sudden and scripted reaction to my question of self-love, he is not as strong in that aspect as he believes himself to be. I also believe that he may need to develop new coping mechanisms, since he stated that he used alcohol to treat his sad or angry emotions. 3. Brief History: The client is a Caucasian male in his late twenties or early thirties, a father of five, and an only child. He was raised by exclusively by his step father since his mother died when he was ten years old. He never knew his biological father. Since his teenage years, he has used alcohol and other substances as a way to deal with negative emotions. Because his step-father worked two jobs in order to provide for him, the client has had little time with his step-father on a personal level. He also claims that one of his biggest concerns is that he believes that he is disappointing his father and not living up to his expectations. He has five children by three different women, has little contact with one of his daughters, lives with two of them full time with his fianc, and has not mentioned the other two. He is in the beginning stages of recovery and has a lot of support from his fianc and her children. 4. Treatment Plan: (Use the Treatment Plan form found in the CETC manual.) 5. Ethical Considerations: The main ethical concern with this client is that his main concern revolves around his addiction recovery process. I do not currently possess the skills and training necessary to address his addiction as an individual entity. Instead, I can only address topics that may be the cause or surrounding the clients addiction.

However, I believe that this can be effective as well because I can target what caused him to resort to alcohol in the first place, thus reducing the chance that hell resort to another substance. 6. Multicultural Considerations: The client is a father and was raised almost exclusively by his step-father. I have little experience with children and no experience with fathers, thus I do not know what he is going through with his children. His upbringing has instilled in him very strict male gender roles, which hindered his emotional development. This is evidenced by how he describes his use of alcohol to regulate his emotions and the strict standards that his father set for him. I was raised exclusively by women, so I was taught to embrace and express my emotions. This may again be a benefit for me in that I can help him learn to embrace his emotions and that society is more accepting of it than he thinks. 7. Summary: The client is a recovering alcoholic, only 30 days into his recovery, and is dealing with a lot of emotions and self-analysis. He believes himself to be doing well in his recover and only wants to talk. However, several topics including self-love, coping mechanisms, accepting negative emotions, grieving the lack of relationship with one of his daughters, and unresolved conflict with his father. I doubt that I would be able to bring resolution to all of these topics, but at least addressing some of these to any extent would help in the other areas. 8. Signature Line:
