Golf Tournament Rules, Terms & Conditions: 1. All golfers will be only allowed to use a 3 wood, 7 iron, pitching wedge and putter. The rest of your clubs will have to remain in your car. 2. We will pay out pro shop credit for the top 7 finishers. All ties will be determined by a card off via the pro shop. 3. Players will play their own ball. Men blue/white tees Women red tees. Golf will be limited to 72 golfers based on order of payments received. 4. All golf entries must be received no later than Friday, April 26th, 2013 5. All Cash sponsorship of the Golf Tournament is payable in full before registration can be confirmed. You will receive a written confirmation. 6. Event Special Features:


1. Fundraisers a. $5 50/50: $5 for 5 tickets. b. $10 Buy a Club: $10 will buy you the opportunity to carry an additional club of your choice. Golfers c. d.
are not allowed to share clubs with other golfers in their foursome. Buy a Stroke: $5 dollars will buy you 5 strokes off your total individual score. Golfers are only allowed a max of 5 free strokes. Casino Holes # 2 & #17, each golfer can wager $5 and if you hit the green you double your money and win $10. All participants will receive a complimentary gift from the Respiratory Disease Society.

Altoona Giant Eagle under the course patio. Driving Range: Complimentary range balls will be provided on the driving range courtesy of 7UP & M&T Bank Putting and Chipping Greens: Practice areas will be open for golfer use before the round courtesy of 7UP & M&T Bank.

Where: Scotch Valley Country Club. 18 Clubhouse Drive, Hollidaysburg PA 16648 About the course: Recognized as one of the finest country clubs in Pennsylvania, it offers a unique challenge on every hole. All playing surfaces have been redone, incorporating bent grasses. Each trap and grass bunker has been relocated or redesigned to provide consistent conditions. Tees and greens have been upgraded and are recognized as some of the fastest, but fairest, greens in the tri-state area. Scotch Valley has been honored to host several major golf events during its history. Included in these are ten qualifying rounds for the United States Open (1994-2003), several state PGA Championships, major college conference golf championships, and a series of charity Pro-Ams that featured many of the PGA's current stars. Course Rating/Slope = 71.2/131 with a par of 72 covering 6,428 yards.
