Your charterer has informed you that a 200t shaft is to be loaded on your general cargo ship. As the chief officer, state the preparations and precautions that are to be taken when loading the shaft.
The preparations and precautions to be taken prior to loading heavy weights are:

Cargo Info from shipper Carry out the Risk assessment prior to commencing the operation to ensure that all possible areas of hazard are taken into account check the ships capacity plan to ensure that there is sufficient space and headroom for the cargo that is to be loaded; Check Rigging plan check the load density for deck or hatch cover on which the cargo is to be loaded on dunnage should be used to spread the load; check for sufficient strong points and lashing materials; Inspect and confirm the lifting points of the load are attached to the load itself. check the SWL of the derrick and ensure that it is adequate; inspect the derrick and overhaul if necessary; check the gears against the chain register, overhaul and inspect to ensure good working condition; test winches, overhaul and inspect carefully; rig according to rigging plans, ensure mast stays are set up taut; reduce free surface within the ship to the minimum thereby ensuring a minimal loss of GM due to free surface effect; ensure that the ship has adequate stability throughout the operation. Describe the requirements in relation to the tests, examinations and inspections requirement in the Register of the ships lifting appliance a s per the ILO Convention No.152, recommendation No.160.
Initial examination and certification Every lifting appliance shall be certified by a competent person before being taken into use for the first time to ensure that it is of good design and construction and of adequate strength for the purpose of which it is intended. Before being taken into use for the first time a competent person shall supervise and witness testing, and shall thoroughly examine every lifting appliance. Every item of loose gear shall, before being taken into use for the first time be tested, thoroughly examined and certified by a competent person in accordance with national law or regulations. Upon satisfactory completion of the procedures indicated above the competent person shall complete and issue the Register of Lifting Appliances and attach the appropriate Certificates. An entry shall be made in Part I of the Register. A rigging plan showing the arrangement of lifting appliances shall be provided. In the case of derricks and derrick cranes the rigging plan should show at least the following information: the position of guys; the resultant force on blocks, guys, wire ropes and booms; the position of blocks; the identification mark of individual items; arrangements and working range of union purchase.
