Title - Procedure for employee Participation, Consultation and

Communication
2.331.1 Introduction

This procedure establishes criteria for Employees Consultation and Communication in reviewing and controlling potential hazards which might be associated with work place and the process operations. HSE& department updates this procedure as and when re!uired" to meet planned Safety targets and statutory re!uirements. 2.331.2 Scope

This is an important critical e-amination techni!ue to identify operation hazards at the design stage of any process. This study is carried out when the design is .frozen/. 't this stage" the potential ma0or hazards should be discovered and the design should be assumed something like in final form. The studies are carried out by a multi disciplinary team. 1ormally the #&+ drawings are referred to the H'()# study. The ob0ective of this study is to identify the hazard at the initial stage of any pro0ect 2design stage3 and take corrective actions. This is applied to any new pro0ects" any ma0or modification in plant" or a section of plant for any change in process or change in process hardware or software . Employees of different discipline are involved in the H'()# studies. Risk assessment #lant safety ambassador and the concerned activity supervisors shall be the team member for risk assessment +f a new chemical is used or new e!uipment is used for the first time or any activity which is one time 0ob and which will continue for more than one day safety ,anager shall also be involved in the risk assessment. Contractor&s supervisor 2in charge of the activity3 shall also be a team member if the 0ob is to be done involving contractors employees Safety committee meetin ,onthly safety Committees will be held in all the plant and facilities and there will be e!ual participation of supervisory and non supervisory employees safety committee meetings shall be displayed on all plant facility notice boards for communication to all employees" safety Committee shall be chaired by H)4 of the plant%Service. Standard !peratin Procedure" Standard #aintenance Procedure Standard )perating #rocedure% Standard ,aintenance #rocedures are re!uired for safe and efficient plant operation" Shop floor employees shall be involved by the plant manager% section head while preparing or reviewing them. Pre-start up safety re$ie% The ob0ective of 5#re Start up Safety *eview is to ensure an inspection prior to commissioning of any facility" plant" e!uipment" etc" after modifications%changes or prior to commissioning of new plant%facility to assure that all such modifications %changes or new plants%facilities are installed in accordance with the intended design "

specification and re!uired safety features are incorporated . The employees of concerned modification" technical services and Safety shall be involved in #S6S*. Incident in$esti ation The concerned employees shall participate in the investigation process as per the procedure +ncident investigation findings% recommendations shall be communicated to all the employees through the plant %dept rack up and the safety committee meetings . #ana ement of c&an e 'll modification shall be initiated as per ,)C procedure. +f the training is re!uired as part of the change it must be imparted and record shall be kept with the ,)C coordinator of the respective plant. 'mer ency plannin and response Employees shall participate in the mock drills conducted in the plant as per procedure They will also participate in the debriefing meeting after the mock drill. Safety #ana ement system audit The Safety ,anagement system will be audited on regular basis to monitor the effectiveness of safety systems. 'udit findings shall be recorded in checklists of the S,S audit.
