ACKNOWLEDGEMENT FORM

The Manchester Monarchs reserve the right to discipline and/or release any Promo Squad Member at any time with or without cause and/or reason. Such decisions will be at the sole discretion of the Manchester Monarchs Organization, Promo Squad Director and/or Manchester Monarchs Human Resources Management and will not be subject to question or review.

Upon completion of preliminary and/or final auditions and considerations for the Manchester Monarchs Promo Squad, I understand that the decision of the new team members will be final, and scores/notes will not be subject to question, review or availability.

I further understand that if I am chosen for the Manchester Monarchs Promo Squad, a full offer of membership/involvement may be contingent upon passing results of a full background check and drug screening.
