WORK PERMIT TRAINING

SAFETY IS THE RESPONSIBILITY OF EACH EMPLOYEE WHAT IS A WORK PERMIT ? A %ork &ermit is a written do ument authorizing a person or a group to perform spe ifi tasks as identified on the %ork &ermit and sub'e t to spe ifi onditions and pre autions. (efore most work in any #A!)* fa ility an be started the workmen must obtain advan e approval in the form of a work permit. *nly one work permit shall be issued for ea h 'ob. %ork performed on a single e+uipment involving more than one work group, ea h group shall get separate work permit. -a h employee or ontra tor who is +ualified will be issued a wallet size authorization ard signed by his .anager or )ontra t Administrator. This ard will list the employee or ontra t employees +ualifi ations and shall be arried on by the person while at work. The authorization ard is valid for a two year period.

&ersonnel may re0+ualify by attending refresher training. All work performed by non0operating personnel and work performed by operating personnel outside the s ope of their normal operating responsibilities shall re+uire in advan e a work authorization by operating personnel in harge of the respe tive area. The #afe and 3azardous work permit will be issued by any authorized issuer from the area where the work is to be done

AUTHORI,ED ISSUER) is an individual who has su essfully ompleted the re+uired training, authorized by his manager to issue work permits, and hold a valid work permit authorization ard. AUTHORI,ED ISSUER RESPONSIBILITIES) 5 )onfirm that the e+uipment and work area are properly prepared for the work to be done and ready for maintenan e. 5 )onfirm the ondition of the area and e+uipment. 5 @o k and tag or onfirm that e+uipment was lo ked out. 5 -4plain to a eptor all hazards asso iated with the 'ob. 5 :isit the 'ob site with the a eptor and issue the permit at the 'ob site. 5 Aotify appropriate personal within the affe ted area. 5 &ost the original opy of the work permit in the ontrol room or the designated area. 5 )ondu t fre+uent visit to the 'ob. 5 Insure that gas tests are performed per the work permit instru tion. $as test fre+uen y must be noted on the safe and hazardous %& by the issuer. AUTHORI,ED ACCEPTOR RESPONSIBILITIES) 5 #hould understand the permits onditions overing the work to be done 5 Review with the issuer the hazards asso iated with the 'ob. 5 )omply with the work permit instru tions. 5 Remain in the area so that he is readily available if needed. If he leaves those areas, another a eptor must endorse the work permit. 5 Aotify the issuer of any work onditions hanges. 5 )lean up the work area and notify the issuer when work is ompleted. 5 :isit the 'ob site with the issuer to lose the work permit, -:-A I! WHAT IS THE DURATION OF WORK PERMITS? 5 5 5 &ermits are valid for only one shift 7C or /2 hours shift1 but may be e4tended up to a total time of 2; hours if endorsed by an authorized issuer and a eptor. &ermits for onstru tion type a tivities in non0operation areas an be issued for periods up to one week. %ork &ermits may be e4tended up to a total time period of /2< hours during the shutdown"turnaround for a spe ifi 'ob and spe ified e+uipment or group of e+uipment su h as welding ma hines, s affolding, et .

HOW TO ENDORSE WORK PERMIT AT SHIFT CHANGE? At $h(%t 1ha34e) the e'#+!"ee 1!'(34 !3 ./t" $ha++ .! the %!++! (34) 5 Review all a tive work permits in his area and endorse the ontrol room opy of the permit. 5 %ithin one hours of shift hange, visit the 'ob site and endorse the hard opy. To issue a work permit for a 'ob in your unit and the 'ob will involve e+uipment in another area, you must he k with the authorized representative in the other area and have him endorse the permit. (34 1!3.(t(!3$) /1 21 91 !ailure to start the work within two hours from the issuan e of the work permit, or if there is an interruption of work for more than two hours. Any deviation without appropriate authorization shall result in immediate suspension of the permit. If operations do not endorse a permit at the work site within one hour.

HOW PLANT EMERGENCY AFFECT WORK PERMITS? All work permits plant wide shall be suspended. %orker shall pro eed to safe assembly areas. WHEN TO RESUME WORK? %hen the DAll )learE is sounded. The issuer or last endorser will insure that it is safe to resume the work. CONFINED SPACE Any area that are not normally o upied by personnel, having limited means of entry, and whi h ould be sub'e t to hazards of o4ygen defi ien y or the a umulation of to4i or flammable gases. GAS TESTING) $as Testing is ondu ted to determine air +uality, work atmosphere suitability and the re+uired personal prote tive e+uipment. (efore an employee is allowed to do gas test with any test e+uipment, he must be +ualified and authorized. GAS TESTING INSTRUMENTS The following instruments are being used for gas testing: /0 .#A 7!ive#tar Alarm1 20 (% $as Alert .i ro 90 >raeger 7will be used by the @ab only1 MSA <F(2eStar A+ar'= This is a portable instrument designed to monitor areas for flammable" ombustible vapor, o4ygen per entage, 32# and )* at the same time. The !ive#tar Alarm ontains sensors, whi h dete t the above gases. *ther sensors an be installed by the @ab to dete t other gases e.g. Ammonia 7A391 The flammable ranges, @-@ J 6-@ are not the same for all ombustible gases. Part) a1 *n"*ff power swit h b1 Reset button 1 &age button d1 Alarm horn and light A11e$$!r") #ampling lines, a 9 meter and 8 meter e4tension hose will be used if needed.
