ONLY students registered with the Centre for Students with Disabilities (CSD) may book their exams with the CSD Exam Centre. Students write exams at the same time that the class is writing unless alternate arrangements have been negotiated in advance and approved by the professor. If the student needs to schedule the test/exam at a different time or date from the class, the professor will be asked by the student to sign an Exam Change Form (bright green in color) granting permission for such a change.
KEEPING US INFORMED
The number of exams that we invigilate has substantially increased. During the 2010/2011 academic year 8922 exams were scheduled in our CSD Exam Centre. We are committed to maintaining the accuracy and integrity of the exam process. However to continue to do this we
need your help. We ask that you please ensure the following is on the test papers that you send to us: Course Code, Course Title, Section Number Name of Professor Date, Time and Length of exam (especially with evening courses) Version number (if multiple versions) Instructions (i.e. no calculators, open book, sheet of notes allowed, permissible texts, student can keep test paper, students arent allowed to leave the class until after a specific time, etc.)
When you provide this information for us, it assists greatly in eliminating confusion for all involved.
COURSE OUTLINES
Please include detailed information on your course outline. In order to know which of their approved CSD accommodations are required for the exam, students need you to provide them with the following information: How long the class will be given to write the exam The date of the quiz/test/in-class assignment/exam What materials are permitted, if any The exam format (multiple choice, short answer, essay, audio component etc.)
If you change the timing of an exam in any way less than a week before the exam we cannot guarantee that the student can be accommodated accordingly. We need the appropriate lead time to arrange for staffing, space, and the provision of the accommodations that the student requires. Students will often assume that lab exams can be written in our Exam Centre. This is not the case. We suggest that you include on your course outline a statement similar to this: CSD registered students must contact the professor directly by [specify a date] to arrange for disability-related accommodation on the lab exam. Due to the exam format and materials required, the lab exams must be written in the lab room.
ON-LINE TESTS
Students must contact the professor directly in order to arrange for extra time for on-line tests and exams. If you are not sure how to add extra time to your online tests, Courseware Support at Ex. 52530 will assist you. Please ask students to provide proof of their registration with the CSD. They may do this by showing you a copy of their CSD Agreement for Services Form or they may email you and copy their CSD Advisor who will provide confirmation of the requested accommodations.
EXAM INVIGILATION
The CSD Exam Centre takes its role as invigilator very seriously. The integrity of the exam process is of utmost concern. Our Centre is staffed with experienced, trained proctors and we have installed windows in the exam room doors both in our group rooms and our private rooms. Proctors check in on students regularly during the exam and exam papers are randomly searched for disallowed materials.
ACADEMIC MISCONDUCT
We have the same problems as other departments do with students occasionally attempting to cheat on exams. In the event that questionable material is found the material is immediately confiscated; the student is instructed to continue writing the exam and the questionable material is returned along with the exam to the professor. We also include with the returned exam, a report describing the incident. It is left to the professor to determine what action is taken with the student afterwards.
