KNOW ALL MEN BY THESE PRESENTS:
I, RAMON DE DIOS., Filipino citizen, of legal age, a widower, a native of Paranaque City, now
actually residing at #17 Aguirre Avenue, BF Homes Paranaque 1700 Metro Manila, being of sound and
disposing mind and memory, and not acting under influence, violence, fraud or intimidation of whatever
kind, declare this to be my Last Will and Testament which I have caused to be written in English, the
language which is known to me. And I hereby declare that:

I wish that by the event that I should die, I would like to be buried according to
the rites of the Roman Catholic Church and interred at our family mausoleum
located at the North Cemetery;

To my treasured children, RAFIKI DE DIOS, SIMBA DE DIOS, JAFAR DE DIOS,

I give and bequeath in equal shares the following:
a. the proceeds of the sale of my house and lot with all the appliances and
furniture and dcor located at #17Aguirre Avenue, BF Homes Paranaque,
Metro Manila with TCT No. 062563 issued by the Registry of Deeds of
Paranaque;
b. the insurance proceeds and all other benefits from my Sun Life Insurance
policy;
To my children and their respective families, I give and bequeath jointly my
summer house located at #15 Winter Street, Brgy. Kaylamig, Tagaytay with TCT
No. 021161 issued by the Registry of Deeds of Tagaytay;
My deposits in Banco De Oro, Bank of the Philippine Islands and United Coconut
Planters Bank amountint to Php 15 Million shall be allotted for the expenses for
the remaining days of my life, the expenses for my funeral, Php 1 Million to be
donated to Philippine General Hospital for indigent patients; and whatever
remains shall be shared by my children equally;
To my alma mater, Polytechnic University of the Philippines (PUP), I hereby
donate a vacant lot for the purpose of building a dorm exclusively for indigent
students coming from the province to be subsidized by PUP located at 143 V.
Mapa Street, Sta. Mesa, Manila;
To my loyal caregiver JOEL MILAN, I give and bequeath the following
a. old furniture of my summer house in Tagaytay; and
b. Php 100,000 cash
To three charitable institutions whose beneficiaries are children afflicted with
cancer, to be determined by my executor, I hereby give and bequeath my
account receivables amounting to Php 2,000,000;
To my executor, I hereby give 1% of the total value of my estate or Php1 Million,
whichever is lower

I hereby revoke, set aside and annul any and all of my other will or testamentary
dispositions that I have made, executed, signed or published preceding this Last
Will and Testament

WE, the undersigned witnesses, whose residences are stated opposite our respective names, do

JOINT ACKNOWLEDGMENT
BEFORE ME, Notary Public for and I the City of San Juan, Philippines, this 7 th day of July, 2007,
personally appeared:
