and 2/28/14.
2. SAVE your receipt(s) from your purchases.
3. FULLY COMPLETE this savings form including name and address.
4. FILL in the TOTAL SAVINGS EARNED box.
5. MAIL the following: this completed form, your original dated cash register receipt and ORIGINAL
UPC barcode(s) from each product package. Note: Exceptions to including the original UPC are
listed on this form. For these items only, you should hand-write the 12-digit UPC code on the form
in the space provided.
CONDITIONS
Product savings limits as shown on the form apply for each promotion offer.
Register receipts must be dated between January 30, 2014, and February 28, 2014. Requests must be postmarked by 3/31/14.
