S.A.F.E.R. /NeoGraft Technique

By Alain CORRE
Every last friday on each month MEDICAMAT organizes a workshop in automated hair transplant technique S.A.F.E.R./ NeoGraft. These workshops are held in our center in Paris and doctors are coming from all over the world to see it in practice. Usually around 10 to 12 doctors and assistants attend the live surgery and get a technical training. They give us a very interesting feedback by the questions asked and the different discussions we have during the lecture. Attending our workshops do not replace the need of an on-site training, but it provides a complete vieuw of the S.A.F.E.R./ NeoGraft technique. Twenty years ago, MEDICAMAT was the rst company to introduce an automated device for Hair Transplant. At that time , the technique was not yet called FUE but Punch Technique. Also, the punch used was not smaller than 1.6mm, which was obviously too big to avoid scars in the donor zone. From that time, with different patents, researches and developments, we have gained in efciency, speed and quality. Year after year, improvements, new techniques and new tools have been developed. Today S.A.F.E.R./ NeoGraft is the less invasive hair transplant technique existing on the market, with almost 0% transection rate. Nowadays, thousands of hair transplant procedures have been carried out with our technique and we were able to achieve a leadership position in the hair transplant market. However with all the evolutions, the hair transplant surgery is still considered as a lenghty procedure. To reduce the time of the surgery, praticioners must get a practical training during which they must know and learn all the essential tricks and techniques. We have been promoting the Strip technique for so many years and I have nothing against this technique. We have worked for many years with our patented Hairtome and multi-blade scalpel but there is no step back. I continue to think that for Hamilton V and above, Strip technique could give a better coverage, meaning more FU in one session but the demand is also changing: As a matter of fact, more than 30% of women are now asking for FUE technique and a good percentage of them wants to

get an eyebrow restoration. The demand is growing for small sessions , which can be repeated over time when FUE technique is used. On the other hand, assistants can be costly and the S.A.F.E.R./ NeoGraft technique can be performed either alone or with one assistant, more specicly for inserting the grafts. I think that a single list as shown below could not be sufcient but at least could help to get always in mind what is compulsory, not only to avoid these problems: time consuming, clogging, bad or slow implantation. But also to be able to provide the best aesthetic results, and to optimize the surgery in terms of comfort and time for you and your patient The small list of dos and donts with S.A.F.E.R./ NeoGraft: - Size of the punch: no more than 1mm except for special cases (0.9mm being a good compromise between 1mm and 0,8mm punch) - Depth of insertion of the punch: not more than 1.5 or 2 mm (when the arector pili muscle is cut, then the graft will be harvested easily by suction). Actually, avoiding transection of the Follicular Unit is the most important thing (a guide can be adapted on the punch to avoid going too deep. See our videos on YouT ube either under the name SAFER or NeoGraft) - Good tumescence in donor zone to increase the angle of shaft of the hairs and also ease the suction - No tumescence in recipient site to avoid frontal oedema - Use only adjustable blade holder to make the slits: grafts must be placed neither too deep nor too supercial: the

best is to make the slit 1mm less than the length of the FU - Before starting, wait a good 5 minutes after making anesthesia to get a proper vasoconstriction: you will get a lot of comfort with almost a bloodless surgery - Keep in mind that 35 hairs/cm2 already gives a nice coverage except for the frontline (a nice frontline of 1cm large requires almost 1,000 FU) - Always give a written instructions to the patient: consent form, pre and post op and make sure that your patient understands them well - Importance of the implanter: many end users who have large experience in manual FUE (or strip) dont feel very condent with our implanters because they have been using tweezers for many years. However, our implanters present many advantages. While using them you never get popping, you are sure that the FU will always be implanted at the same depth. Also, there is no squeezing or damage to the bulge area. Besides, reimplantating with the implanters takes the exact same time as with the tweezers. For the record, in Japan I have seen Guinness record of 950 FU inserted in 1 hour with our implanters! This has been said, in like any surgery, there is a learning curve to follow and you should take time slowly to get the same speed you could have with tweezers. Indeed, there are so many other things that are important in automated FUE and accordingly dont hesitate to revert to me: Ill be glad to share our large experience with you. Dont forget that we offer on-site trainings for you and your assistant!
