Worthington Hills CC, Medallion Club, Scioto CC July 1518, 2014 ELIGIBILITY Entrants must be Daughter or Son or Ward of a full member of a CDGA Afliated Club. CDGA GHIN# must be on application. RULES USGA rules will govern all play except for local rules approved by CDGA. FORMAT 36 Hole Stroke play qualier on Tuesday and Wednesday at Worthington Hills CC.

The top 16 boys regardless of age will begin match play on Thursday at Medallion Club. The top 4 boys 15 & under who did not make championship bracket will begin match play on Thursday at Medallion Club. The top 4 girls will begin match play on Thursday at Medallion Club. Final matches are on Friday at Scioto CC

ENTRY Entry is subject to approval or rejection at any time by CDGA Tournament Committee.
No practice round is included in entry. Entrants must be 18 or under on 7/15/14

ENTRY FEE $35.00. Entry includes lunch each day for players. Fee will not be refunded
after deadline unless player submits a written medical excuse. or e-mail entries.

DEADLINE July 1, 2014, 4 p.m. (or when eld lls at 120). No Tee Entries. No Fax, phone CODE OF CONDUCT Players will adhere to a strict "CODE OF CONDUCT' and those who
do not compete as gentlemen and ladies, without profanity, tantrums and other outbursts will be disqualied. Parents are subject to similar standards.

CADDIES NO CADDIES PERMITTED. Parents must remain a reasonable distance from play,
at least 50 yards. After the rst warning, the player will be disqualied.

DRESS CODE A proper dress code will be enforced all four days. NO JEANS, CUTOFFS

OR RUNNING SHORTS. Ball caps will be worn correctly, and shirts tucked in. No caps will worn indoors at anytime. Players who fail to follow this code are subject to disqualication.

Entrant must be a Daughter or Son or Ward of a full member of a CDGA affiliated club. Check made out to CDGA must accompany this application.

TEE TIMES Requested tee times can not be guaranteed. Copies of starting times will be sent to
CDGA Pro Shops approximately one week in advance of event. Contestants may also check OHIOGOLF.ORG for tee times.

HAMILTON HOUSE
The Columbus District Golf Association will send $5 per player from each entry fee to the Capital Campaign for the new Evans Scholar House at the Ohio State University. The new house, located at 52 East 14th Avenue is named the Hamilton House in honor of Earl Bones Hamilton. $5 per player of the entry fee qualies as a charitable donation for federal income tax purposes.

EVANS SCHOLARSHIP
Evans Scholars Foundation administers the nations largest privately funded college scholarship program. Since 1930, over 8,500 deserving young caddies have earned their college degrees through Evans Scholarships. Today, more than 820 caddies attend college each year as Evans Scholars. The Evans Scholarship was founded by golf great Charles Chick Evans Jr. (1890-1979).
