ThistechnicalmemohasbeenpreparedinresponsetotheCitysrequestforasummaryofthe eventsofFebruary2021,2014,itsrootcauseandthepathforwardfromatechnical perspective. Summaryofevents Thesinkholeeventwas,ascouldbesurmised,unexpected.OttawaLightRailTransit Constructors(OLRTC)hadnoindicationsofconcern,andnonoteddeviationsfrompracticeprior totheevent.Miningoperationswereproceedingaccordingtoapprovedplansanddrawings. Dailyrecordsconfirmedthatgeotechnicalconditionscloselymatchedgeotechnicalassumptions. Yet,atabout09:40pmonFebruary20th,theminingcrewnoticedexcessivelooseandwet materialstarttofall,betweenthetunnelsupportpipes,fromtheleftsideareaofthetunnel crownontothetunnelfloor.Thismaterialcontinuetoingressand,despiteeffortstostabilizeit frombelow,onlystabilizedafterasinkholedeveloped.Adjacentroadwayswereassessedand, at10pm,crewsclosedWallerStreetnearLauriertolocalvehicularaccess.Atabout1:00amon February21stthesinkholereachedthesurfaceinanOLRTCworkzoneonWallerStreet. DamagefromthesinkholewaslimitedtotheCityutilities(sewerandtrafficsignals)thatcrossed its path. Extensive monitoring points OLRTC have in the area have confirmed that no building settlement of concern occurred. OLRTC via the applicable Engineers of Record provided technical guidance during the stabilization exercise and the safety report confirms that no workers or other persons were injured as a result of this event. OLRTC crews worked together withtheCity tomanage thetrafficandtransiteffects andby2:00amonFebruary22OLRTC had stabilizedandbackfilledthesinkhole. Rootcause Sincethestabilization,OLRTChasreviewedallpertinentdataandpreparedarootcause technicalanalysisreport.Thereportconfirmsdocumentedprocedureswerefollowed, appropriatematerialswereused,prescribedsupportingmeasureswereinplaceandtherewere nononconformancesofpractice. Thereportdoesidentifythatthetunnelcrossedbeneathapreviouslyexcavatedconstructionpit thatwasdeeperthantheknownutilitiesandlikelyextendedfrombedrocktothesurface.The
