paid premium production goals.

The above symbols represent the levels of PAC.
Business persistency and business quality are
also considered for entry and continued participation
in the PAC. Members of the PAC are subject to
business review by Home Ofce representatives.
You will begin receiving benets the rst day of the
month after qualifying for the PAC. Benets do not
begin or are not applied until after you have qualied.
PAC benets are based on production after your start
date and are not retroactive based on production
prior to your start date.
Once you qualify, benets continue ONLY for the
current calendar year and the calendar year after
qualifying. You can re-qualify for the PAC to receive
benets past the second calendar year.
For example, if you qualify in September 2014, you
receive qualied benets for the remainder of 2014
and the whole calendar year of 2015. Your production
level for 2015 determines the PAC qualication level
for 2016. In this example, not meeting minimum
production goals during 2015 will discontinue
membership in the PAC for 2016.

As a Platinum or Diamond member you are upgraded from the Premier Service Group
to a Personal Case Manager who will be your single point of contact for you and your staff.
Youll have direct access to your Personal Case Manager who is responsible for all aspects
of new business from start to nish and is able to answer inquiries regarding in force policies.
In this one-on-one relationship, your case manager understands your business model and
can therefore tailor the services they provide to better support you.

change periodically. Interpretations of the rules and nal determination of Premier Agent Club qualication and any benets of the Premier Agent
Club will be at the sole discretion of Midland National

. We reserve the right to modify, discontinue, deny qualication, or remove agents from
Premier Agent Club program at any time. All decisions are nal. The Agent Contract shall remain in full force and otherwise govern the rights of
the parties. Termination of the Agent Contract will automatically terminate agents participation in the Premier Agent Club. Agents must agree
to business review and/or seminar review at the request of Midland National

. Except as expressly provided herein, this information does not


amend, modify or change the terms of the Agent Contract.
Qualication: Qualication is based on all Midland National

xed annuity business. Renewal premiums added to in-force annuity contracts will
count towards Premier Agent Club qualication as long as the annuity contract was originally issued within ve years. Internal Replacement
premiums will not qualify for Premier Agent Club program. Production cannot be accumulated between agents. Agent will be responsible for
value of all awarded items by issuance of an IRS 1099 Form when applicable.
1. Commission: This commission bonus is payable only to the writing agent on personal production applications on rst year paid premium
only. Paid premium for commission bonus is dened as paid premium minus rst-year cancellations, withdrawals or surrenders. First-year
renewal premiums added to in-force annuity contracts will receive the Commission Bonus as long as the original contract was issued after
the agent was formally placed in the Premier Agent Club. Subagent business will not be eligible for the bonus. For licensed-only agents, the
commission bonus will be paid to the commission-assigned entity. Commission bonus will be paid through normal commission channels.
This commission bonus is payable on a monthly basis and will be paid no later than the 20th of the subsequent qualifying month. Any debit
balance incurred by the agent/agency will reduce any applicable commission bonus. Products eligible for commission bonus will change as
the Midland National

product portfolio changes and are subject to state availability. Agents should keep informed of current products and
state availability by contacting Sales Support or visiting www.MidlandNational.com.
2. Substitutions for the Travel Reward are not allowed.

4. Agent must obtain approval and submit proof of Practice Management or Client Event expenses incurred by sending receipts to
Midland National

Annuity Divisions home ofce.
5. Payment of Annual Production Bonus is paid to the writing agent in a lump-sum amount following the end of the calendar year.
