Registration 1.1 You can download the application from the SLT website www.slt.lk or collect the same from your nearest SLT Regional Telecom office or SLT Teleshop 1.2 Registration can only be done by submitting a dully filled application in hard copy to ; 1.2.1 Relevant STL Account Manager by SLT Corporate customers And ; 1.2.2 SLT Regional Telecom office or SLT Teleshop by other customers

Since the online registration facility is not yet available.

Define the customer Please state the following information and attach the required documents in answering item 01 on the General Information. For Company Name of the Company (Attach certificate of incorporation under Act No.07 of 2007) For Partnership Name of the partnership and the full names of the partners For Sole Proprietorship Name of the Sole Proprietorship and the full name of the owner (Attach Business Registration in both cases and NIC copies)

Contact information 3.1 Contact details of the Legal owner or the authorized person should be mentioned under the item No.2 on contact information. 3.2 Access to the service shall only be done by the password holder, namely the legal owner stated under 2.1 and the authorized person of the business customer stated under 2.2. The password shall not be shared by the password holder with any other person.

Capacity of Authorized person to select Authorized Access Level/s 4.1 In recommending Authorized Access Levels please refer the note 02 in the application or contact SLT officers for further information.

4.2 The Regional Telecom Office Manager will consider as to whether the authorized person

whose information is stated under item 02.02 in the Application form will have the capacity to select the Authorized Access Levels. Such applications will be entertained only upon the approval by the Regional Telecom Office Manager. 5. Signing 5.1 For residential customers only the legal owner of the telephone line can sign this application. Any other persons request will not be entertained unless the same is made along with a consent letter signed by the legal owner. 5.2 In case of business customers, only the Applications signed by the partners themselves or the sole proprietor himself will be accepted. In the Case of a company, only the Applications with the company common seal will be accepted. 5.3 Power of Attorney Holders Power of Attorney holders signing for and on behalf of any of the Customers should submit a copy of the Power of Attorney and an Affidavit on the validity of the Power of Attorney. 5.4 Authorized person (for Business Customers only) If an Authorized person is signing on behalf of the company, attach the resolution passed for that. If one partner is signing for all the partners attach the letter of authorization signed by all partners. 5.5 Any Application signed by a Third party, without proper authorization will be rejected by SLT, since the information made available are strictly confidential highly personal.

Password Change 6.1 An e-mail stating your default password will be sent to the email address stated by you under the item no.02 on contact information in the application. 6.2

Forthwith the initial login to WSC, you should change the default Password. Further, it is strongly recommended to change the password from time to time.
