Introduction

PGEO Group is wholly owned by Wilmar International Limited, one of Asia's largest integrated agribusiness groups. PGEO Groups main activity is the manufacturing of edible oils. Its activities are vertically integrated, beginning with the conversion of crude oils into refined oil products to the marketing of these oil products in bulk, steel drums and consumer packs for the local and international markets. It has one of the largest refining capacities in the country with eight refineries in Peninsular and East Malaysia. Annually, PGEO Group refines more than 4 million metric tonnes of edible oils and about 90% are exported overseas to China, India, Unites States, EU countries, Middle East, Pakistan, Japan, Philippines, Australia and New Zealand. In addition, PGEO Group is also actively involved in crude palm oil futures trading, soyabean and palm kernel crushing, oleochemical manufacturing, steel drums manufacturing, manufacturing of calcium salt for animal feed and other downstream activities like the production of shortening and hydrogenated products, cocoa butter replacer and other specialty fats. On top of that, PGEO Group has diversified into the renewable energy industry beginning with the operations of biomass steam generation plants to supply steam, electricity and chilled water to three of its factories. Subsequently, it marked its presence in the biodiesel industry with the commencing of a biodiesel plant in January 2007. In November 2013, it further diversified with the acquisition of a chemical bulking installation in Pasir Gudang. PGEO Groups commitment towards excellence has striven itself to continuously im prove and brings only the best service to its customers.

Analysis of the article

On 17/01/2014 10:30AM Mohd Ridzuan Bin Hassan worked as a plant operator there has been an accident at workplace when after using steam to melt the frozen oil, victim go to steam valve to close it. He go to the location where the steam valve on the side of fat trap. The victim felt into the fat trap containing hot water and hot oil while stepping on the unstable platform to shut off steam valve which located at fat trap. An accident happened due to the contractor worker put back the fat trap cover wrongly ( horizontally instead of vertically ) after completing the desludging job. All bodt parts ( tummy and below ) suffered hot burn but he managed to raise and flush himself at the nearby shower. His colleagues and security guard En Zaaba sent the victim to Hospital Regency Masai using company's vehicle immediately and he was warded for better treatment.

Route cause & hazard

The causes of these incidents occurred because of mistakes from employer who are fail to implement available safety equipment and proper procedures for a project. As a result of this mistake, hazards are easy to get by the employee. Among other factors are the factors that lead to the creation of the environment occurs. Stepping on unstable platform caused the accident occurs in the workplace. At the same time the victim does not use safety equipment such as safety harness. There is some hazard discovered near the place of work which is down from the platform, unsafe acts, unsafe conditions. When employees work in Platform was not put back correctly by contract workers after taking it . When fall it can result in death. To act insecure about when employees work nonchalant they may endanger themselves because they can get the pain to themselves. Unsafe conditions are when employees work in unsafe areas such as many holes, working at non stable platform and so on. It can cause death and pain to themselves.

Personal Responsibility

Employer In OSHA 1994, Part IV Section 15 (1), as their employer should provide safety while working as proper personal protective equipment. Supervisor As a supervisor he should be sure employees work in a safe manner and in a safe place to work.

Legal Requirement
FMA (Safety Health and Welfare Regulation 1970), Regulation 12. Where any person is require to work at a place from which he will be liable to fall a distance of more than ten feet, means shall be provided to ensure his safety and such means shall where practicable include the use of safety belts or ropes. FMA 1967, Regulation 23 Effective provision shall be made for securing and maintaining sufficient and suitable means to protect any person employed from exposure to the element, and such means may include roof shelters and wind break for protection against rain, and wind respectively OSHA 1994, Part IV, Section 15 (1) It shall be the duty of every employer and every self-employed person to ensure, so far as is practicable, the safety, health and welfare at work of all his employees

Recommendation

Installing a fence in the fat trap. Turning off platform that can not be lifted and moved and made a hole in the platform Cut off steam pipe into fat trap Installing safety board Employers are required to provide security facility equipment such as safety harnesses Management should consider allowing employees to stop work if they are aware of any unsafe conditions.

Conclusion
The conclusion is the accident happen because of lacking awareness in safety. The management refuses to participate in implement safety at the workplace. Safety is everybody responsibility. Both employer and employee have their own responsibility in ensure safety. Moreover, our country already had law regarding safety that must to be complying by all the workers and employer. Although there are highly risks working environment, we can came out with some recommendation in concern to avoid the accident and minimize the risk.
