COMPARATIVE OF PEDICLE SCREW PLACEMENT ( A LABORATORY EXPERIMENTAL STUDY)


Abstrak :

Study Design. Comparative observational analysis study of pedicle screw placement between first degree and second degree of orthopaedic residens in Airlangga University-Dr Soetomo teaching hospital. Objectives. To compare accuracy of pedicle screw placement between first degree and second degree of orthopaedic residens in Airlangga University-Dr Soetomo teaching hospital in fresh cadaveric spine. Summary and Background data. There are many injury of the spine and with trauma cases increased every year. Most of trauma cases is traffic accident. One of the tool to perform stability of the spine post trauma is pedicle screw. Pedicle screw is one of orthopaedic surgeon competency. That’s why orthopaedic residens should ever and can perform this operative procedure. There are so many equipment to decreased complication of pedicle screw placement, but not every hospital have this kinds of tool. The most important is basic anatomy of pedicle, as Roy – Camille has performed his technique. Methods. Fourteen residens from first degree and ten residens from second degree has performed pedicle screw placement in eight fresh cadaveric spine without image intensifier. There were two hundred and thirty two screw which devided in three spine segments, upper thoracic, lower thoracic and lumbal segment. The accuracy of placement is based on AP, lateral and axial view of computerized radiologic, observed by a second degree resident and a orthopaedic surgeon. Results. There are no difference between first degree and second degree residen in upper thoracic spine. But there were statistically difference between first degree and second degree residen in lower thoracic and lumbar spine, with second degree resident were more accurate than first degree resident. Conclusions. Although there were statistically difference between first degree and second degree residen in lower thoracic and lumbar spine, with second degree resident were more accurate than first degree resident, but overall the accuracy still lower than Roy – Camille expectation, 90 % of all pedicle screw placement.
