BIG IDEA: A community is a group of people who live, work, learn, and play together. MISSION: All members of a community deserve every opportunity to have a quality of life that involves support, training, and connection with a specific purpose in order to be a productive citizen. THE TEAM: Open and direct invitations will be extended to the following groups: Parents and students impacted by disabilities and who are involved in middle, high school and/or secondary programs. School district administrators, personnel and related service people, along with Special Education teachers, paraprofessional educators and general education teachers. Professionals from the local county education service centers and community service organizations Other important members will include local business owners and representatives from local non-profit agencies who work with individuals impacted by disabilities. THE CREWS: Each group mentioned above will be represented on crews in order to share the responsibilities of creating, implementing and supporting a smooth transition for our students identified with disabilities from school to career. Connection Crew Empower students through a clear and structured assessment & evaluation process and design a program in educating and training students, families and community. Communication Crew Promote the transition program through the use of various forms of printed and technical media, as well as, create awareness opportunities for the local community and school district. Collaboration Crew Identify and recruit various local business owners, community service and non-profit agencies, and educational institutions in building partnerships for job training, authentic work experiences, and potential job/careers. THE GOAL: To develop a quality transition program that involves students, parents, educators, and the community in career exploration, education, and work experience for students and young adults with disabilities. THE OBJECTIVES (suggestions & ideas to start): The Connection Crew will Survey students and families to identify SPINS. Identify educational needs in order to design programs that meet the needs of all involved. Create and present training and awareness programs to families, students, and community The Communication Crew will Identify effective ways to communicate the program across the groups (school, home, work) Establish meaningful communication to build awareness Design and implement communication timeline to promote the work of other crews The Collaboration Crew will Explore possible businesses and agencies to match with student SPINS Coordinate referrals with community work sites and work experience programs Link community agencies with families that will provide support and guidance in the process
