DIGOXIN
Cardiac Glycoside Elixir

> Drug of choice

for CHF. > Decrease myocardial workload. > Slow HR in sinus tachycardia due to CHF.

Increases the force and velocity of myocardial contraction (positive inotropic effect) by increasing the refractory period of the AV node & increasing total peripheral resistance.

> Ventricular fibri-

> headache llation > weakness > heart block >drowsiness > acute MI >n&v >renal insufficiency > diarrhea > electrolyte ab> dizziness normalities > anorexia > presence of Digo> visual Xin toxicity disturbance > hypokalemia

1. Risk for
Imbalanced Fluid volume Related to Increased renal perfuSion secondary to Effects of the Drug. 2. Ineffective Tissue perfusion related to change in cardiac output. 3. Impaired Gas exchange r/ to changes in Cardiacoutpu

1. If taking
non-potassium Sparing diuretics as well as Digoxin, will need potassium supplements. 2. If gastric distress experienced, use antacid. 3. Take after meals to lessen gastric irritation.

MONTELUKAST
Leukotriene Receptor Antagonist Singulair

> prophylaxis & Chronic treatment ofasthma > relief of symptoms of seasonal allergic rhinitis. > relief of symptoms of perenial rhinitis. > Prevention of Exercise-induce BronchoconsTriction.

Cysteinyl leukotrienes and leukotriene receptor occupation are associated with symptoms of asthm Including airway edema, smooth muscle contraction, and inflammation. Montelukast binds with cysteinyl leukotriene receptors thus preventing the action of cysteinyl leukotrienes.

> used cautiously in pts. With hepatic or renal impairment. > not indicated for the treatment of acute asthmatic attacks.

> headache 1. Acute Pain r/to head> dizziness ache,GI > diarrhea upset. > abdominal pain 2. Risk for > fever Injury r/to > nausea & CNS effects vomiting
3. Deficient Knowledge regarding drug therapy.

1. Caution the patient that these drugs are not to be used during an acute asthmatic attack. 2. Instruct client to continue drug durin acute attacks as well as during symptom free periods. 3. Administer drug on an empty stomach,1 hr before or 2hrs after meals.

NITROGLYCERIN
Vasodilator Nitro-bid Nitrostat

Drug of choice for treatment of Acute angina attacks.

HINDI TO KASALI
CELECOXIB NSAIDs Drug

Nitrates relax and dilates veins, arteries & capillaries allowing increased blood flow through the vessels & lowering systemic blood pressure. The vasodilation causes blood to pool in veins & capillaries, decreasing preload, while the relaxation of the vessels decreases after load. This effects greatly reduces the cardiac workload and the demand for oxygen, bringing the supply and demand ratio back to balance.

> severe anemia > head trauma or Cerebral hemorrhage > pregnancy or Lactation > pts. with hepatic Renal dse. > hypotension

> > > >

headache dizziness weakness nausea & Vomiting > flushing of The skin > pallor > increased perspiration

1. Decreased Cardiac output r/t vasodilation and hypotension effect. 2. Risk for Injury r/t CNS or cardiovascular effects. 3. IneffecTive Tissue Perfusion r/t hypotension.

1. Do not chew Or crush capsules-swallow whole. 2. Stand slowly Because drug May cause dizziness, lightHeadedness,or fainting. 3. Use acetaminophen to relieve headache without reducing the medications.

Celebrex
> Relief of S/S of osteoarthritis & rheumatoid arthritis in adults. > Acute pain in adults. > Primary dysmenorrhea. Inhibits prostaglandin synthesis, primarily by inhibiting cyclooxygenase-2 (COX2), thus decreasing inflammation. > Presence of allergy to sulfonamides. > Cardiovascular dysfunction > Peptic ulcer or known GI bleeding. > Pregnancy and lactation > Diarrhea > Nausea and vomiting > Abdomi-nal pain > Flatulence Constipation >dyspepsia 1. Acute Pain r/t CNS and gastrointestinal GI eff. 2. Ineffective Breathing Pattern if Toxic occurs 3. Deficient Knowledge Regarding Drug therapy. 1. take this drug with food to decrease stomach upset. 2. Avoid alcohol cause it may aggrava-te liver function. 3. Report any S&S of liver toxicity.

BUDESONIDE GlucocortiCoids

Entocort EC

> for relief of Symptoms of Seasonal allerGic rhinitis > an oral agent for the treatment of mildto moderate active Crohns Disease.

Glucocorticoids enter target cells and bind to cytoplasmic receptors, initiating many complex reactions that are responsible for antiInflammatory and Immunosuppressive Effects.

> allergy to any steroid > presence of any acute infection > lactation > pts.with diabetes

> headache > tremor > rash > fainting

1. Disturbed Sensory Perception 2. Ineffective Coping r/t body changes caused by the drug 3. Decreased Cardiac Output r/t Fluid reteNtion.

1. Do not used to treat Acute asthma Attacks. 2. Take oral tablets whole; do not crush.
